斑点追踪成像技术对冠心病患者室壁运动的二维应变研究

摘    要:目的利用二维斑点追踪成像技术测量心室肌局部长轴应变(LS)、圆周应变(CS)及径向应变(RS),分析2D-STE诊断局部心室肌运动异常的临床意义.方法随机选取根据冠脉造影结果确诊的冠心病患者为60例,分为轻度及中重度冠脉狭窄组,冠脉狭窄50%的30例患者为对照组,测量3组患者心肌各节段收缩期峰值应变.结果 1)中重度狭窄组LS,RS和CS均小于对照组及轻度狭窄组(P0.01).2)LS轻度狭窄组与对照组相比较,心尖段及中间段差异具有统计学意义(P0.01).中重度狭窄组前壁心尖段与对照组比较差异具有统计学意义(P0.05).3)CS轻度狭窄组与对照组比较,心尖段差异具有统计学意义(P0.05).中重度狭窄组与对照组比较,后间隔心尖段、前壁中间段及前壁心尖段差异具有统计学意义(P0.05),前间隔中间段及心尖段差异具有统计学意义(P0.01).4)RS轻度狭窄组与对照组比较,前间隔心尖段差异具有统计学意义(P0.05),后间隔心尖段、前间隔基底段及中间段、前壁基底段及中间段差异具有统计学意义(P0.01).RS中重度狭窄组与轻度狭窄组比较,前壁基底段差异具有统计学意义(P0.05),前间隔中间段、前壁中间段及心尖段差异具有统计学意义(P0.01).结论二维斑点追踪技术可敏感评价心肌运动功能变化,从而评估冠脉狭窄程度.

Abstract:Objective To measure minister axial strain, circumferential strain and radial strain of local ventricular muscle by two-dimensional speckle tracking imaging technology,to analyze 2D- STE in the diagnosis of movement abnormal clinical significance of local ventricular muscle. Method This study randomly selected 60 patients with coronary heart disease according to the results of coronary angiography, they are divided into two groups:mild and moderately severe coronary stenosis group, the control group ( 30 cases, coronary stenosis <50%). The myocardial segmental systolic peak strain is measured. Results 1) LS,RS and CS in moderately severe stenosis group were less than those of the control group and mild stenosis group ( P<0 . 01 ) . 2 ) LS mild stenosis group compared with control group,the difference of apex section and the middle section was statistically significant ( P<0 . 01 ) . The difference of anterior wall apex in moderately severe stenosis group was statistically significant comparing with the control group ( P<0 . 05 ) . 3 ) The difference of apex section of CS mild stenosis group was statistically significant comparing with control group ( P<0 . 05 ) . Moderately severe stenosis group compared with control group,the difference of the interval period nowadays,anterior wall after the middle period of the anterior wall and apex section was statistically significant (P<0. 05),the difference of interval before the middle segment and the apex section was statistically significant ( P<0 . 01 ) . 4 ) RS mild stenosis group compared with control group,the difference of the interval before the apex section was statistically significant(P<0. 05),the interval before the middle period and apex section was statistically significant ( P<0 . 01 ) . RS moderately severe stenosis group compared with mild stenosis group, the difference of the anterior wall basal segment was statistically significant (P<0. 05),the difference of interval before the middle period,among the anterior wall and apex section was statistically significant ( P<0 . 01 ) . Conclusion Two-dimensional spot tracking technology can assess myocardial movement function change extent of coronary stenosis.
