子午流注针法配合辨证取穴对缺血性脑血管病患者脑血流动力学的影响

目的观察子午流注针法配合辨证取穴对缺血性脑血管病(ICVD)患者脑血流动力学的影响。方法将120例ICVD患者随机分为2组,治疗组60例采用子午流注针法配合辨证取穴针刺治疗,对照组60例采用辨证取穴针刺治疗。2组均10 d为1个疗程,治疗3个疗程。观察2组临床疗效及针刺前后椎动脉(VA),基底动脉(BA),大脑前动脉(ACA)、中动脉(MCA)、后动脉(PCA)的平均血流速度(Vm)及动脉搏动指数(PI)变化,并观察临床疗效。结果治疗组临床疗效优于对照组(P0.01)。2组治疗后ACA、MCA、PCA、VA及BA的Vm均较本组治疗前显著增快(P0.01),且治疗组显著高于对照组(P0.05)。治疗组治疗后PCA、VA、BA的PI值与本组治疗前比较均有明显降低(P0.05,P0.01),且治疗组低于对照组(P0.05)。结论子午流注针法配合辨证取穴针刺治疗ICVD可提高临床疗效,改善脑动脉弹性,促进脑血管扩张,增加脑血流量,有利于神经功能的恢复。

Observation of the effect of midnight - noon ebb - flow acupuncture therapy combined with point selection treatment based on syndrome differentiation on cerebral hemodynamics index in ischemic vascular patients

Objective To observe the effect of midnight - noon ebb - flow acupuncture therapy combined with point selection treatment based on syndrome differentiation on cerebral hemodynamics index in patients with is-chemic cerebrovascular disease(ICVD). Methods 120 ICVD patients were randomly divided into two groups. 60 patients in treatment group were treated by midnight - noon ebb - flow acupuncture therapy combined with point selec-tion treatment based on syndrome differentiation. 60 patients in control group were treated acupuncture based on syn-drome differentiation. The course was 10 d in two groups for 3 courses. The clinical effect,the variation of mean flow velocity(Vm)and arterial pulsatility index(PI)of vertebral artery(VA),basilar artery(BA),anterior cerebral artery(ACA),middle cerebral artery(MCA)and posterior cerebral artery(PCA)before and after treatment were observed in two groups. Results The curative effect in treatment group was superior to that in control group(P ﹤0. 01). The mean flow velocity of ACA,MCA,PCA,VA and BA after treatment were significantly increased in two groups(P ﹤ 0. 01),and the increase in treatment group was obviously higher than that in control group(P ﹤ 0. 05). The PI of PCA,VA and BA after treatment were significantly decreased in two groups(P ﹤ 0. 05,P ﹤ 0. 01),and the decrease in treatment group was lower than that in control group(P ﹤ 0. 05). Conclusion Midnight - noon ebb -flow acupuncture therapy combined with point selection treat-ment based on syndrome differentiation can enhance the clin-ical effect on ICVD,improve cerebral artery elasticity,can promote cerebral vasodilatation, increased cerebral blood flow which is advantageous in the nerve function restoration.
