摘    要:目的 观察细胞因子白细胞介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)在糖尿病(DM)患者发病过程中的变化,以及谷氨酸脱羧酶抗体(GAD-Ab)和胰岛素自身抗体(IAA)的存在状况.方法 采用放射免疫分析(RIA)法和免疫放射分析(IRMA)法,对200例DM患者和50例正常对照者血清中IL-6、TNF-α、GAD-Ab和IAA进行检测.结果 DM患者血清IL-6和TNF-α含量均较正常健康者高(P<0.05).在DM1与DM2两组中,DM1组IL-6值显著高于DM2组和正常对照组(P均<0.01);TNF-α在DM1与DM2两组中其含量均高于正常对照组,但以DM2组较为显著(P<0.05).在DM患者中,DM1组GAD-Ab的阳性检出率为75.0%,明显高于DM2组和正常对照组(P<0.01);DM2组IAA阳性率高于DM1组(P>0.05).结论 IL-6和TNF-α在DM的发病机制中有十分重要的意义,检测DM患者血清IL-6和TNF-α含量,对于判断DM患者的免疫状态,以及胰岛和胰岛素的功能状态具有重要意义.GAD-Ab、IAA联合检测对于提高DM诊断的准确性和自身免疫性1型DM的检出率、减少误诊率与漏诊率具有重要价值.

Abstract:Objective To investigate the pathogenesis change of cytokine interleukin -6 (IL- 6) and tumor necrosis factor- α(TNF- α) in diabetes mellitus (DM), and to investigate the existing condition of glutamic acid decarboxylase antibody (GAD - Ab) and insulin autoantibodies(IAA). Melhods Serum levels of IL- 6, TNF- α, GAD- Ab and IAA were detected by radioinmmnoassay (RIA) and immunora- diometric assay methods (IRMA) in 200 cases of DM patients and 50 normal controls. Results Serum levels of IL- 6 and TNF- a in DM patients were higher than those in healthy (P 〈 0.05). Serum level of IL - 6 in DMI group was significantly higher than that in DM2 group and control group (P 〈 0.01 all) ; Serum level of TNF - α in DM1 and DM2 groups were higher than that in control group, but that in DM2 group was high significantly (P 〈 0.05). Positive rate of GAD - Ab in DM1 group was 75.0%, which significantly higher than those in DM2 group and normal control group (P 〈 0.01 ); IAA- positive rate in DM2 group was higher than that in DM1 group (P 〉 0.05). Conclusion It was important to study the pathogenesis of DM by detecting IL - 6 and TNF - α. Levels of IL - 6 and TNF - α could be used as judging the patient' s immune status, functional status of insulin and insulin. Combined detection of GAD- Ab and IAA could improved the accuracy of diagnosis of autoimmune type 1 DM, reduced the misdiagnosis rate and the rate of misdiagnosis.
