奥氮平联合氟西汀治疗难治性抑郁症临床观察

目的：评价奥氮平（悉敏）联合氟西汀治疗难治性抑郁症的疗效和安全性。方法：60例难治性抑郁症患者随机分为联合组和对照组，联合组采用氟西汀20mg／d＋奥氮平5mg／d，对照组单纯给予氟西汀20mg／d，疗程8周。采用汉密尔顿抑郁量表17项（HAMD，7）评价临床疗效。结果：除对照组治疗1周后HAMD。评分改变不明显以外，两组在治疗后1周、2周、4周和8周时HAMD，评分均较基线值有明显降低，差异有统计学意义（P〈0．05）；联合组在治疗后1周、2周、4周和8周时HAMD17评分均低于对照组，差异有统计学意义（P〈0．05）。联合组和对照组的总有效率分别为80．0％和53．3％，差异有统计学意义（P〈0．05）。联合组和对照组不良反应发生率分别为43．3％和36．7％，差异无统计学意义（P〉0．05）。结论：奥氮平联合氟西汀治疗难治性抑郁症临床疗效高，并且不良反应与单用氟西汀差异不大。

The effect of olanzapine plus fluoxetine in the treatment of treatment-resistant depression

Objective：To evaluate the efficacy and safety of olanzapine plus fluoxetine in the treatment of treat- ment-resistant depression.Methocls：60 cases with treatment-resistant depression were randomly divided into joint group and control group. The control group given only fluoxetine 20 mg/d, and the joint group were treated with olan- zapine 5 mg/d additionally. They were all performed for 8 weeks. The efficacy evaluated by Hamilton Depression Scale 17 item （HAMDI7）. Results ：Compared to baseline, the HAMDI7 scroes were all decreased in the two groups by the end of 1, 2, 4, 8 week （or weeks） except 1 week in the control group（P〈0.05）. In all those investigation points the HAMDl7 scroes in joint group were all lower than those in control group （P〈0.05）. The total effective rate in joint group was 80.0%, higher than 53.3% in control group （P〈0.05）. The adverse reaction ratio was similar between the two groups （43.3% vs 36.7%, P 〉0.05）. Conclusion ：h is an effective therapy of olanzapine plus fluoxetine in the treatment of treat- ment-resistant depression, and the adverse reactions remain at an acceotable level and are the same in the two grouos.
