摘    要:目的:探讨CT和MRI对颈静脉孔区神经鞘瘤的诊断及鉴别诊断。方法:回顾性分析了10例颈静脉孔区神经鞘瘤的CT和MRI影像资料。CT检查6例,其中4例作增强扫描,MR检查10例,其中8例作增强扫描,结果:根据CT和MRI表现颈静脉孔神经鞘瘤分为A,B,C三型,A型4例,B型1例,C型5例。神经鞘瘤易发生囊变,注射造影剂后肿瘤不均匀强化。结论:CT与MRI相结合,能更全面地为临床提供诊断,鉴别诊断及治疗所需要的信息,为治疗方法的选择提供可靠的依据。

Abstract:Objective:To study the value of CT and MRI in the diagnosis,differential diagnosis of jugular foramen schwannomas.Methods:CT and MRI study of ten patients with jugular foramen schwannomas were reviewed retrospectively.Six cases were evaluated with CT,among them,four cases were examined by contrast CT studies.Eight cases were examined with precontrast and postcontrast T1-weighted MR imaging and with noncontrast T2-weighted MR imaging.Two were scanned without contrast study.There were four women and six men (mean age 41.8 years) with a symptom duration ranging from six months to four years(mean 1.5 years).Results:Depending on the CT and MRI features,the tumors were classified into three types:Type A,a tumor primarily at the cerebellopontine angle with minimal enlargement of the jugular foramen(four cases);Type B,a tumor primarily at the jugular foramen with intracranial extension (one case);Type C,a primarily extracranial tumor with extension into the jugular foramen (five cases).Jugular foramen schwannomas displayed cystic changes,and showed inhomogeneous enhancement after administration of contrast medium.Conclusion:CT combined with MRI can accurately provide all the informations necessary to clinical diagnosis,differential diagnosis as well as selecting therapeutic methods.
