重度单侧感音神经性耳聋患者听觉传导通路激活的fMRI研究

摘    要:目的 研究重度单侧感音神经性耳聋患者给予声音刺激后,听觉传导通路各核团的激活范围及强度.方法 耳聋组和正常听力对照组的双耳分别给予刺激,将被试分为4组.并进行fMRI数据处理与分析.结果 ①给予刺激后,4组被试听觉传导通路的各组核团可见不同程度的激活;②4组被试的听觉皮层、内侧膝状体、下丘在4000 Hz和500 Hz两种纯音刺激下的激活情况存在差异.结论 ①听觉皮层在500 Hz比在4000 Hz纯音刺激下的激活范围及强度大;②内侧膝状体、下丘激活范围变化不大,激活强度变化率500 Hz大于4000 Hz;③单侧感音神经性耳聋患者的听觉传导通路发生结构与功能的重组,重组发生于听觉皮层水平.

Abstract:objective To study the activation range and intensity of each nucleus of the auditory pathway under sound stimulation in patients with severe unilateral sensorineural hearing loss. Methods The hearing-impaired and normal-hearing control groups were stimulated to the left and right ear separately. The subjects were divided into 4 groups and tested. The fMRI data were then processed and analyzed. Results ①Given the stimulation, the 4 groups showed different degrees of activation in the nuclei of auditory pathway. ②There were significant differences in the activation of auditory cortex, medial geniculate body and inferior colliculus of the 4 groups under the 500 Hz and 4000 Hz pure-tone stimulation. Conclusion The activation range and intensity of auditory cortex under 500 Hz stimulation were greater than those under 4000 Hz stimulation. The activation range of medial geniculate body and inferior colliculus changed little while the change rate of activation intensity at 500 Hz was higher than that at 4000 Hz. The functionat and structural reorgan)zations occurred in the auditory pathway, specifically in the auditory cortex.
