氟比洛芬酯脂微球静脉预处理预防异丙酚注射痛的临床研究

摘    要:目的对比研究氟比洛芬酯脂微球或利多卡因静脉预处理预防异丙酚注射痛的效果。方法将静脉麻醉下行宫颈锥切术的66例患者随机均分为生理盐水组(A组)、1%利多卡因组(B组)和氟比洛芬酯脂微球组(C组),在近心端距穿刺点10cm处结扎止血带,静脉注射药物和2min。后松开止血带,随后靶控输注异丙酚达效应室质量浓度3.2μg/mL。从注射异丙酚开始到患者意识消失为止,每隔10s询问输液侧手臂是否出现疼痛,评估用药依从性。结果B组与C组患者视觉模拟评分(VRS)及用药依从性差异无统计学意义,但VRS评分均明显低于A组,用药依从性均明显高于A组。3组患者水肿、静脉炎和药疹的发生率差异无统计学意义。结论氟比洛芬酯脂微球和利多卡因静脉注射均可以安全有效地预防异丙酚注射病

Abstract:Objective To investigate the efficacy of flurbiprofen axetil or lidocaine preceded by venous occlusion for reducing pain of propofol injection. Methods In this prospective and controlled study, sixty- six patients scheduled to undergo elective cervical conization under total intra venous anaesthesia (TIVA) were randomly allocated into three groups. In group A, the patients received sodium chloride injection. Group B was administrated with lidocaine injection. Group C received flurbiprofen axetil injection. All patients pretreated with drugs preceded by manual venous occlusion with a rubber tourniquet for 2 min and followed by administration of propofol target controlled infusion (TCI), injected into the largest vein of the hand through IV cannula. During propofol injection (terminated at unconsciousness),a researcher who was blinded to treatment assignment asked each patient about her pain at the injection site every 10 s. Responses were scored on a 4- point scale, recording the compliance of patient. Results No significant differences between group B and C were found. A significantly higher proportion of patients in group A experienced pain compared with the group B and group C. The median pain intensity scores were signif- icantly lower in the group B and group C compared with group A. The incidence and intensity of propofol- induced pain were statistically similar between tire group B and group C. There were significantly higher with respect to the compliance of patient in group B and C than in group A. Conclusion Flurbiprofen axetil or lidoeaine is effective in reducing propofol- induced pain on injection.
