摘    要:目的采用应变率成像技术与多普勒超声心动图对比研究,定量评价肺动脉高压(PH)患者右心室舒张功能。方法选取肺动脉高压患者31例(PH组)及正常对照组33例,计算两组三尖瓣口舒张早期血流速度与三尖瓣环运动速度之比(E/e’),应变率成像技术获得右心室各个节段舒张期纵向应变率及运动速度。结果与对照组相比,肺动脉高压组三尖瓣舒张早、晚期血流速度之比(E/A)、三尖瓣口舒张早期血流速度与三尖瓣环运动速度之比(E/e’)差异有统计学意义(P<0.05);两组相比右心室游离壁及室间隔各段舒张早期应变率(SRe)有显著性差异(P<0.05);右室游离壁及室间隔舒张早期应变率(SRe)与三尖瓣口舒张早期血流速度与三尖瓣环运动速度之比(E/e’)、右室游离壁厚度、肺动脉收缩压相关性较好。结论应变率成像技术是评价肺动脉高压患者右室舒张功能的较好方法。

Assessment of right ventricular diastolic function in patients with pulmonary hypertension using strain rate imaging echocardiography

Abstract:Objective To evaluate right ventricular diastolic function using strain rate imaging echocardiography in patients with pulmonary hypertension and compare with the results of routine parameters of Doppler echocardiography.Methods This study included 31 cases of pulmonary hypertension patients and 33 healthy cases regarded as control group.E/e' ratio between tricuspid flow and tricuspid annulus diastolic movement speed was calculated.Longitudinal strain rate and velocity of each segment of right ventricular were measured by strain rate imaging at diastolic phase.Results Compared with the control group,systolic strain rate at early diastolic phase,E/A and E/e' were significantly high (P<0.05) ; The early-diastolic strain rate between each segment of right ventricular free wall and ventricular septum were statistically different (P<0.05) ; The average early-diastolic strain rate of right ventricular free wall and ventricular septum were correlated well with Pulmonary artery systolic pressure and the thickness of right ventricular free wall.Conclusion Strain rate imaging would be a potential way to evaluate right ventricular diastolic function in patients with hypertension.
