| 摘 要： | 目的:探讨持续肾脏替代治疗(CRRT)重症感染患者应用万古霉素的药学监护方法。方法:临床药师参与1例CRRT重症感染患者的万古霉素监护实践,辅助医师进行药物剂量和给药方案调整。结果:对CRRT患者进行万古霉素血清药物浓度测定,可以优化万古霉素剂量调整方案。结论:临床药师通过开展万古霉素血清药物浓度测定,能够提高万古霉素剂量调整的合理性。

| Abstract: | OBJECTIVE： To study the pharmaceutical care of vancomycin for CRRT （continuous renal replacement therapy） pa- tient with severe infection. METHODS： Clinical pharmacist assisted physician to adjust the dose and medication program, based on the monitoring of vancomycin for a CRRT patient with severe infection. RESULTS： The serum concentration of drug was deter- mined so as to optimize the dose of vancomycin and adjust drug program. CONCLUSION： Clinical pharmacists help to improve the rationality of vancomycin dose adjustment through determination of vancomycin concentration in serum. |
