骨髓增生异常综合征患者CD34、CD95的表达及临床意义

摘    要:目的研究骨髓增生异常综合征(MDS)患者骨髓CD34^+细胞比率及CD34^+细胞CD95(Fas)表达率并探讨其临床意义。方法应用流式细胞术检测26例MDS患者骨髓单个核细胞(BMMNC)中CD34^+细胞比率及CD34^+细胞Fas表达率。结果MDS患者CD34^+细胞比率RAEB、RAEB—T组较RA组、正常对照组显著增高(P〈0.01);RAEB、RAEB-T组间比较差异有显著性(P〈0.01);RA组与正常对照组之间则无显著性差异(P〉0.05)。CD34^+细胞Fas表达率RA、RAEB、RAEB—T组均较正常对照组显著增高(P〈0.01);RA与RAEB、RAEB—T组间比较差异有显著性(P〈0.01);RAEB与RAEB—T组间比较则无显著性差异(P〉0.05)。结论CD34^+细胞比率进行性升高系提示MDS病情进展的标志;CD34^+细胞Fas表达率在MDS显著增高但于疾病恶性转化过程中其表达率逐渐降低;细胞增殖与凋亡异常可能系MDS主要发病机制之一。

Expression of CD34 and CD95 in patients with myelodysplastic syndrome and their clinical significance

Abstract:Objective To study the bone marrow CD34^+ cell ratio and expression of CD95 (Fas) of CD34^+ cells in patients with myeledysplastic syndrome (MDS) and to explore their clinical significance. Methods The ratio of CD34 ^+ cells in bone marrow mononuelear cells(BMMNC) and CD34^+ cell Fas expression in 26 patients with MDS were detected by using flow cytometry along with labelled CD34 and CD95 monoclonal antibodies. Results The ratio of CD34^+ cells in patients with MDS-RAEB and RAEB-T were much higher than that in RA and control group ( P 〈 0. 01 ). There was a significant difference between RAEB and RAEB-T group( P 〈 0.01 ), but there was no difference between RA and control group ( P 〉 0.05 ). The expression of Fas of CD34^+ cell in RA, RAEB and RAEB-T group significantly increased compared with that in the control group ( P 〈 0.01 ). The CD34^+ cell' s Fas expression in RAEB and RAEB-T group were significantly higher than that in RA group( P0.01 ), while there was no significant dif- ference ( P 〉 0.05) between RAEB and RAEB-T group. Conclusion The increased ratio of CD34^+ cells in MDS patients may be a indicator of disease progression. The expression of Fas of CD34 ^+ cells significantly increase in MDS patients and gradually decrease in the process of malignant transformation. Abnormal cell proliferation and apoptosis may be a possible principal pathogenesis of MDS.
