摘    要:目的探讨急性出血坏死性胰腺炎(ANP)时中性粒细胞(PMN)聚集于肺脏的机制。方法在ANP并发急性肺损伤(ALI)模型,采用免疫组化技术等方法,动态观察ANP后大鼠肺脏实质细胞细胞间粘附分子1(ICAM1)、PMN表面巨噬细胞分化抗原1(CD11b/CD18)的表达改变和PMN肺脏聚集的变化。结果ANP的肺脏ICAM1表达逐渐增加,与PMN肺脏聚集程度呈显著正相关,而PMN表面CD11b/CD18在ANP后1小时即迅速表达至高峰,并持续24小时。结论ANP后,PMN在肺脏的大量聚集是建立在PMN表面CD11b/CD18与肺实质细胞ICAM1高表达的基础之上,而ICAM1为这一粘附过程中的可变因素,是影响PMN粘附并聚集于肺脏的主要因素。

Abstract:Objective To explore the mechanism of polymorphonuclear neutrophil (PMN) infiltration in lung after acute hemorrhagic and necrotizing pancreatitis. Methods Adopting a model of rat ANPinduced acute lung injury (ALI), We observed the dynamical changes of ICAM1 expression in lung tissue and CD11b/CD18 expression on PMN and PMN infiltration in lung tissue. Results The ICAM1 expression in lung gradually increased, which significantly correlated with the PMN accumulation after ANP. PMN expressed CD11b/CD18, which quickly occurred and achieved the peak at 1 hour and later maintained high expression within 24 hours. Conclusion PMN infiltration in lung is based on increased expression of ICAM1 in lung tissue and CD11b/CD18 on PMN after ANP. Furthermore, ICAM1 may be a principal factor in the changes of the PMN infiltration in lung tissue and adhesion between PMN and VEC after ANP.
