补肾活血汤配合牵按推挤手动复位治疗肾虚血瘀型桡骨远端骨折临床研究

目的:观察自拟补肾活血汤配合牵按推挤手动复位治疗肾虚血瘀型桡骨远端骨折的临床疗效及对患者骨密度和骨折愈合情况的影响。方法:选取2018年3月至2019年3月本院收治的桡骨远端骨折患者120例,中医辨证均为肾虚血瘀证,按照随机数字表法分为观察组和对照组,每组60例。所有患者均给予牵按推挤手动复位联合小夹板固定术治疗,观察组在此基础上给予补肾活血汤治疗。结果:观察组治疗后骨密度和骨代谢指标高于对照组,差异有统计学意义(P<0.05);观察组治疗后中医证候评分低于对照组,差异有统计学意义(P<0.05);观察组Gartland-Werler评分、PRWE评分及骨折愈合时间均低于对照组,差异有统计学意义(P<0.05);观察组有效率为98.33%,对照组有效率为78.33%,观察组高于对照组,差异具有统计学意义(P<0.05)。结论:自拟补肾活血汤配合牵按推挤手动复位联合小夹板固定术治疗肾虚血瘀型桡骨远端骨折,可显著改善患者临床症状,缩短骨折愈合时间,促进骨的形成和腕关节功能恢复。

Clinical Study on the Treatment of Distal Radius Fracture of Kidney Deficiency and Blood Stasis Syndrome with Decoction of Tonifying Kidney and Activating Blood Circulation Combined with Manual Reduction of Traction by Pulling and Pushing

Objective:To observe the clinical curative effect of Self-Made Decoction of Tonifying Kidney and Activating Blood Circulation combined with manual reduction of traction by pulling and pushing on distal radius fracture of kidney deficiency and blood stasis syndrome and the influence on bone mineral density and fracture healing.Methods:A total of 120 patients with distal radius fracture treated in our hospital from March 2018 to March 2019 were selected and divided into the observation group and the control group,with 60 cases in each group according to the method of random number table.All the patients were treated with manual reduction of traction by pulling and pushing combined with small splint fixation,and the observation group was additionally treated with Self-Made Decoction of Tonifying Kidney and Activating Blood Circulation.Results:The bone mineral density and bone metabolism index of the observation group were significantly higher than those of the control group,and all the differences were statistically significant(P<0.05).After the treatment,the TCM syndrome score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).The Gartland-Werler score,PRWE score and fracture healing time of the observation group were lower than those of the control group,and all the differences were statistically significant(P<0.05).The effective rate of the observation group was 98.33%,which was higher than that of the control group was 78.33%,and the difference was statistically significant(P<0.05).Conclusion:Self-Made Decoction of Tonifying Kidney and Activating Blood Circulation combined with manual reduction of traction by pulling and pushing has a remarkable clinical curative effect on distal radius fracture of kidney deficiency and blood stasis syndrome,can significantly improve the clinical symptoms,shorten the time of fracture healing,promote the formation of bone and the recovery of wrist function.
