长期卧床患者肺部感染病原菌耐药性分析及治疗策略

目的:了解长期卧床患者肺部感染的病原菌情况,探讨其细菌耐药性及治疗策略。方法:通过对我院2008～2009年绝对卧床在10 d以上的住院患者120例进行跟踪治疗,分析其肺部感染病原菌分布特点及耐药率。结果:120例长期卧床引起肺部感染的患者,痰标本共培养出致病菌257株,革兰阳性菌占22.6%,革兰阴性菌占75.9%。金黄色葡萄球菌、鲍曼不动杆菌、铜绿假单胞菌、大肠埃希菌、肺炎克雷伯菌所占比例较大,耐药率高。结论:引起长期卧床患者肺部感染的细菌耐药率高,治疗过程中应做好药物选用工作。

Pathogens Drug Resistance Analysis and Treatment Strategies in Long-term Bedridden Patients with Pulmonary Infection

OBJECTIVE： To probe into pathogens conditions of long-term bedridden patients with pulmonary infection and to investigate bacterial drug resistance and treatment strategies. METHODS ： By tracking the treatment of long-term bedridden patients in our hospital from 2008 to 2009, the distribution of pathogen and the rate of drug resistance in long-term bedridden patients with pulmonary infection were analyzed. RESULTS： A total of 120 long-term bedridden patients suffered from pulmonary infection. 257 strains of bacteria were cultured by sputum specimens. Gram-positive bacteria accounted for 22.6 % and gram-negative bacteria accounted for 75.9%. Staphlococcus aureus, Acinetobacter baumanmi, Pseudomonas aeruginosa, Escherichia coli and Klebsiella pheumoniae got the largest proportion and high drug resistance. CONCLUSION： The bacteria which cause pulmonary infection in long-term bedridden patients have the high rate of bacterial drug resistance. The selection of drugs in the treatment process should be taken carefully.
