静脉用蔗糖铁治疗腹膜透析患者肾性贫血25例疗效观察

目的探讨持续性非卧床腹膜透析患者应用静脉补铁纠正肾性贫血的临床疗效。方法选择2010年3月至2011年3月收治的非卧床持续性腹膜透析的患者50例,随机分为静脉补铁组(静脉组)和口服补铁组(口服组)各25例。两组患者均给予促红细胞生成素(EPO)3 000U/次,皮下注射,每周2次,口服叶酸片30 mg/d。在此基础上静脉补铁组给予蔗糖铁注射剂(森铁能),采用分次输入的方法;口服补铁组给予琥珀酸亚铁(速力菲)0.3克/次,3次/d。观察两组患者血红蛋白(Hb)、红细胞数(RBC)、红细胞压积(HCT)、血清铁蛋白(SF),并计算出转铁蛋白饱和度(TSAT)。结果两组患者在治疗后贫血均有改善,但静脉组改善情况明显优于口服组,两组比较差异有统计学意义(P<0.05)。结论促红细胞生成素治疗持续性非卧床腹膜透析患者贫血,通过静脉补铁较口服补铁能更好地纠正贫血,且能明显提高患者的生活质量。

Effect of intravenous iron sucrose in treatmen of peritoneal dialysis in patients with renal anemia 25 cases

Objective To investigate the clinical efficacy of the continuous ambulatory peritoneal dialysis patients with intravenous iron to correct renal anemia.Methods A hospital in March 2010-March 2011 were treated continuous ambulatory peritoneal dialysis,50 patients were randomly divided into intravenous iron group(Group IV) and oral iron group(oral group) 25 cases in each.Two groups of patients were treated with erythropoietin(EPO) 3000U / times,subcutaneously,twice per week,oral folic acid tablets 30mg/d.On this basis,intravenous iron group was given iron sucrose injection(Forest Railway to),the use of sub-sub input methods;Oral iron group received ferrous succinate(Philippines-speed power) 0.3g/time,3 times / day.Observed in patients with hemoglobin(Hb),red blood cells(RBC),hematocrit(HCT),serum ferritin(SF),and calculate the transferrin saturation(TSAT).Results The two groups of patients were improved after treatment of anemia,but the improvement was significantly better than oral group,the difference was statistically significant(P<0.05).Conclusions Erythropoietin treatment of continuous ambulatory peritoneal dialysis patients with anemia,compared with oral iron supplementation by intravenous iron to correct anemia can be better,and can significantly improve the quality of life of patients.
