肝纤维化定量评估肝硬化病人肝功能储备力的临床研究

目的 了解肝纤维化定量对评估肝硬化病人肝功能储备力的价值。方法 对19例正常对照组和23例肝炎后肝硬化肝病组病人术前测定MELD评分，同时采用计算机辅助数字图像分析法检测正常对照组和肝病组病人肝组织标本的纤维化程度，分析肝纤维化定量评估与Child-Push分级及MELD评分间的关系。结果 肝硬化患者肝纤维化面积百分比与MELD评分间呈正直线相关关系（r=0．84，P〈0．01），Child A、B、C3组间肝纤维化面积百分亦具有显著性差异（P〈0．01）。结论 肝纤维化定量评估是评估肝硬化患者肝储备功能的有用方法之一。

Clinic study of hepatic functional reserve in patients with viral liver cirrhosis by using quantitative assessment of hepatic fibrosis

Objective To investigate the value of quantitative assessment of hepatic fibrosis in predicting hepatic functional reserve in patients with viral liver cirrhosis.Methods By calculating MELD score of 23 patients with viral hepatic cirrhosis and 19 normal controls before operation and evaluating quantitative values of fibrosis of liver specimens after operation using computer-assisted digital image analysis,correlation among MELD score and Child-Turcotte-Pugh classification and the percentage of hepatic fibrosis was analyzed.Results The positive linear correlation(r=0.84) was found between the values of the percentage of hepatic fibrosis and MELD score in patients with viral liver cirrhosis,and the correlation was significant at the 0.01 level.Differences of the values of the percentage of hepatic fibrosis between three groups(Child-pugh A,B,C classification) were statistically significant(P<0.01).Conclusion In patients with virus-related cirrhosis,quantitative assessment of hepatic fibrosis can be used to predict the reserve of hepatic function.
