| 摘 要： | 目的探讨儿科ICU(PICU)呼吸机相关性肺炎(VAP)的影响因素及病原菌的分布特点,为预防控制VAP提供参考。方法对医院PICU2008年1-12月机械通气患儿进行目标性监测,将持续机械通气48 h的患儿纳入研究对象,应用多元Logistic回归方法确定VAP影响因素。结果共收集病例132例,其中发生VAP 33例,发生率为25.0%;分离到病原菌15株,以革兰阴性菌为主,占75%;Logistic回归分析显示,机械通气前肺炎(OR=0.07,P0.001)、机械通气持续时间5 d(OR=4.58,P0.05)、重复插管(OR=3.68,P0.05)、外科手术(OR=3.74,P0.05)为VAP的独立影响因素。结论 PICU患儿是发生VAP的高危人群,应加强PICU的VAP监测与控制。
