摘    要:以头孢噻肟、头孢他啶、头孢曲松和氨曲南为指示底物,用双纸片协同实验、纸片表型确证实验、三维实验和E-test法4种方法检测45株产超广谱β-内酰胺酶(ESBLs)阳性和59株产ESBLs阴性的大肠埃希菌和肺炎克雷伯菌.结果发现,4种方法的敏感性依次为95.56%,97.78%,93.33%,97.78%,特异性依次为96.61%,100.00%,98.31%,100.00%;头孢噻肟为宁夏地区用于检测ESBLs的最适指示底物;纸片表型确证实验和E-test法是4种检测方法中用于检测产ESBLs较可靠的方法,用三维实验检测产ESBLs菌株适合标本量较少的小型医院,双纸片协同实验和纸片表型确证实验在所有医院都适用.

Abstract:To Detect 45 strains of producing extended-spectrum beta-lactamase (ESBLs) and 59 strains nonproducing ESBLs in Escherichia coli and Klebsiella pneumoniae with double-disc synergy test, disc diffusion confirmatory test, three-dimensional test and E test, ceftazidime,cefotaxime,ceftriaxone and aztreonem were used as indicators. The sensitivity was 95.56% ,97.78% ,93.33% and 97.78% ,the specificity was 96.61% ,100.00%, 98.31%and 100. 00%, respectively. Cefotaxime was considered to be the better indicator. The disc diffusion confirmatory test and E-test were more reliable methods. The double-disc synergy test and disc diffusion confirmatory test were more convenient and suitable for the routine screening in hospital.
