血浆置换治疗原发性巨球蛋白血症的临床研究

利用CS3000血细胞分离机对4例原发性巨球蛋白血症行血浆置换疗法。每次置换量2500～3500ml，共进行17次。血浆置换前后测定Hb、Hct、WBC、BPC、ESR、IgA、IgG、IgM。结果显示每次血浆置换后血清IgM平均下降33．3％，血浆置换量与血清IgM水平呈负相关。在同等置换量情况下，置换前血清IgM水平越高，置换后血清IgM下降越明显，作者根据结果对血浆置换治疗原发性巨球蛋白血症的适应症及治疗方法进行讨论。

A clinical study of plasma exchange in the treatment of waldenstrom's macroglobulinemia

4 patients suffering from waldenstrom's macroglobulinemia were treated with plasma exchange (PE). The dual Chamber continous instrument (CS-3 000 Blood Cell Separator, Fenwal Laboratories , Deefied, IL) was used and the amount of plasmapheresis was from 2 500 to 3 000 ml each time. 17 PE were carried out. Before and after plasma exchange the following parameters were determined: Hb, Hct, WBC, plat, ESR, serium immune globin (IgA, IgG, IgM). The result and shown that there were a 33. 3% (medium) drop in serium IgM each PE and negative correlation between the level of serium IgM and the amount of plasmapheresis. The level of serium IgM was the higher before PE and the lower after PE on condition that same amount of plasmapheresis was performed.
