彩色多普勒超声对肝硬化的肾脏血流变化的研究

为研究肝硬化时肾脏血流变化情况。对48例普通肾功能检测正常的肝硬化患者及31例对照组作多普勒超声主肾动脉、段间动脉、叶间动脉阻力指数（RI）测定。结果表明：肝硬化组RI使明显高于对照组（P＜0.01）;Child-pugh计分肝功能B级及C级RI值高于A组（P＜0.01或＜0.05），B级与C级间差别无统计学意义（P＞0.05），与对照组比较，除A级的段间动脉外（P＞0.05），A、B、C级RI值均高于对照组（P＜0.01或0.05）;少量或无腹水组及大量腹水组间RI值无差异（P＞0.05）。上述结果提示普通肾功能正常的肝硬化患者已有了肾脏血流的改变。多普勒超声有可能应用于肝硬化时肾功能损害的病理生理学及临床研究。

Color Doppler Flow Imaging for Study Renal Blood Fow in Patients with Liver Cirrhosis

To evaluate the renal blood flow perfusion in patients with liver cirrhosis when renal function tests are normal, tha main, the segmental and the interlobar renal arterial RI were measured for 48 liver cirrhotic patients and 31 control by Deppler ultrasound. The results revealed RI was significantly higher in cirrhotic patients than the control. There was no RI difference between mild, non-ascitic and severe ascitic Patients. Renal hemedynamic changes might occur in liver cirrhosis patients even the renal function tests were normal. Doppler ultrasound may be useful for early recognization of renal functional impairment in liver cirrhosis.
