脓毒症大鼠心功能的变化

摘    要:目的:研究脓毒症大鼠离体心脏心功能的变化。方法:12只雄性SD大鼠随机分为假手术组(n=6)和脓毒症组(n=6),脓毒症模型采用盲肠结扎穿孔术(CLP),术后12h检测血清肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、丙氨酸转氨酶(ALT)及天冬氨酸转氨酶(AST)的水平,确定脓毒症模型是否成功。术后12h取出心脏,置于langendorff离体灌流装置上,记录灌流10 min、20 min、30 min、40 min、50 min、60 min的心率(HR)、收缩期左心室内压上升的最大变化速率(+dp/dtmax)、舒张期左心室内压变化的最大速率(-dp/dtmax)、左室内压差(△LVP=左室收缩压-左室舒张压)及测定冠脉流量(CPF)。结果:与假手术组比较,心率未有显著改变(P〉0.05),但+dp/dtmax、-dp/dtmax、△LVP及CPF均出现显著下降(P〈0.05)。结论:CLP诱导的脓毒症大鼠离体心脏早期即有心肌损伤的存在,主要表现为收缩和舒张功能的下降。

Abstract:Objective:To investigate the changes of ex vivo myocardial function of rats with sepsis. Method: Twelve male SD rats were randomized into sepsis group and control group. The rats of sepsis group,receive cecal legation perforation (CLP). At 12 hours after operation, the biochemical index including CK-MB, CK, ALT and AST were detected to define the sepsis model. The isolated rat heart was perfused in a Langendorff apparatus at 12 h after operation. Heart rate (HR), left ventricular developed pressure (ALVP),+dp/dtmax,- dp/dtmaxand coronary perfusion flow (CPF) were measured at 10 min, 20 rain, 30 min,40 min, 50 min, 60 min after perfusion. Result: Compared with control group, myocardial functional parameters (~LVP, + dp/dtmax , -- dp/dtmax) and coronary flow (ml/min) of sepsis rats were significantly (P〈0.05) reduced, but no significantly change was observed in the HR (P〉0.05). Conclusion: Myocardial injury developed at early stage of sepsis in isolated ratsmainly manifested as decreased myocardial systolic and diastolic functions.
