磁共振弥散张量成像在脑膜瘤诊断中的应用

摘    要:目的探讨磁共振弥散张量成像在脑膜瘤诊断中的应用价值。方法选择60例脑膜瘤患者为研究对象,均接受颅脑MRI平扫及DTI检查,测量并比较肿瘤实质区、瘤周水肿区及瘤周白质区ADC值及FA值。结果1I级脑膜瘤中,上皮细胞型比例最高,为40%,其次为纤维型,比例为28%。各病理亚型ADC值比较,血管瘤型最低,为(7.55±1.17)×10-9 mm2/s,微囊型最高,为(14.24±2.43)×10-9 mm2/s。2良性脑膜瘤中,瘤周水肿ADC值最高,与肿瘤实质及瘤周白质比较,差异具有统计学意义(P0.05)。瘤周白质FA值最高,显著高于肿瘤实质及瘤周水肿,差异具有统计学意义(P0.05)。恶性脑膜瘤中,瘤周水肿ADC值最高,与肿瘤实质及瘤周白质比较,差异具有统计学意义(P0.05)。瘤周白质FA值最高,显著高于肿瘤实质及瘤周水肿,差异具有统计学意义(P0.05)。良恶性脑膜瘤比较,恶性脑膜瘤肿瘤实质ADC值及瘤周白质FA值均显著低于良性脑膜瘤,差异具有统计学意义(P0.05)。结论 DTI能够显示脑膜瘤实质及瘤周组织特征,可以为其诊断和治疗提高有价值的信息。

Abstract:Objective To explore the application value of diffusion tensor magnetic resonance imaging in the diagnosis of meningioma.Methods 60 cases of meningioma patients as the research object,all Underwent brain MRI scan and DTI examination,ADC value and FA value measurement of tumor region,peritumoral edema region and peritumoral white matter were detected and compared.Results 1The proportion of benign meningiomas epithelial cell type was the highest 40%,followed by fiber type,ratio of 28%.Comparison of ADC values,the lowest for hemangioma type,(7.55±1.17)×10-9 mm2/s,Microcapsule was highest,(14.24±2.43)×10-9mm2/s.2 Compared with the tumor and peritumoral white matter,benign meningioma peritumoral edema of the highest ADC value,the difference was statistically significant(P〈0.05).The highest FA value of peritumoral white matter,Significantly higher than that of tumor and peritumoral edema,The difference was statistically significant(P〈0.05).Compared with the tumor and peritumoral white matter,the ADC value of Malignant meningioma peritumoral edema were higher,the difference had statistically significant(P〈0.05).the FA value of peritumoral white matter was significantly higher than that of tumor and peritumoral edema,The difference had statistically significant(P〈0.05).Comparison of benign and malignant meningiomas,Malignant tumor of ADC value and FA value of peritumoral white matter were significantly lower than those of benign meningiomas,the difference was statistically significant(P〈0.05).Conclusion DTI can display the essence and characteristics of meningioma peritumoral tissue,Can improve the valuable information for its diagnosis and treatment.
