| 摘 要： | 目的分析股骨近端形态改变的原因，指导术中操作，减少并发症发生，提高手术疗效。方法2000年1月至2008年1月对30例伴有股骨近端形态改变的患者行全髋关节置换术。术前根据Berry分类系统，按照导致畸形的原发病因、畸形存在的部位、畸形的几何形态进行多重分类，对应进行影像学检查，设定手术方案。术后以Harris评分、放射学改变、步态、肢体长度、Trendelenburg征评定治疗效果。结果患者均得到随访，时间5个月至8年。平均3年4个月。Harris评分由术前平均48．3分恢复到术后平均89．4分。术后患者双下肢等长，Trendelenburg征均阴性，截骨端骨愈合率达91％、1例出现骨不连。结论根据术前详细分析股骨近端形态改变的原因，制定相应的手术计划，可有效的避免术中操作不当或不利，减少术中、术后并发症的发生，提高手术疗效。
