公交车乘员群体伤中颌面创伤救治分析

目的:分析城市公交车乘员群体伤中颌面创伤的临床特征,积累急诊救治经验。方法:回顾所诊治的7批公交车群体伤中颌面创伤乘员89例临床资料,总结其临床特征和急救方法。结果:全部伤员均Ⅰ期治愈。坐位组乘员主要为撞击伤,口腔颌面软组织伤和牙外伤较多,颌骨骨折较少发生;站位组乘员为撞击伤和摔伤,颌骨骨折和合并伤多见,病情较重。结论:公交车乘员群体伤中颌面创伤的严重程度与乘员在车箱中所处位置有直接关系。因其损伤对乘员容貌和心理创伤较大,不能忽视群体伤救治中颌面创伤的处理。

Analysis and emergency treatment of bus passengers with maxillofacial injuries from traffic accidents

Objective:To analyse the clinical characters of maxillofacial injuries in bus passengers from traffic accidents.To build up experiences in the emergency treatment of maxillofacial injuries.Method:Retrospectively analyse 89 patients with maxillofacial trauma from 7 groups of bus passengers in traffic accidents.The clinical characters and emergency treatment were discussed combining with literatures.Result:The whole victims were cured.The majority of sitting passengers suffered from impact injuries.Injuries in maxillofacial soft tissues and tooths frequently occurred in sitting passengers,while maxillofacial fractures were less frequent.Standing passengers suffered from impacting injuries and tumbling injuries.Maxillofacial fractures and associated injuries were more frequent in standing passengers and their pathogenetic condition were severe.Conclusion:The severity of maxillofacial trauma is directly associated with the positions of bus passengers.The maxillofacial injuries cause great damages to the facial features and psychic trauma,thus proper treatment on maxillofacial injuries should never be ignored when traumatic population occurs.
