癫患者长时程脑电图与CT检查的相关性研究

目的:探讨长时程脑电图(LTME)和CT在癫诊断、鉴别诊断和病因诊断中的作用。方法:本文分析2005年1月~12月采用LTME(SOLARROVER-8型AEEG和SOLAR2000型VEEG)及GE1800型CT对320例癫患者的检查结果。结果:LTME正常123例(38.4%),异常197例(61.6%)。临床发作48例(15.0%),同时占异常脑电图的24.4%。异常脑电图中样波114例,占异常的57.9%,非特异性异常83例(42.1%)。在LTME中,AEEG和VEEG的异常率及临床发作率均无明显差异,x2=1.32,P>0.05及x2=0.93,P>0.05。CT正常229例(71.6%),异常91例(28.4%)。CT与LTME两组异常相比,有非常显著性差异,x2=10.88,P<0.001。结论:LTME的临床应用,大大提高了癫脑电图的阳性率,特别对无明显临床发作表现和先兆指征的可疑性癫患者进行诊断和分类。CT广泛应用使脑部疾病引起的癫阳性率大为提高,降低了原发性癫的诊断率,已成为癫病因研究的重要手段,它能寻找癫最可能和最重要的潜在原因。CT与脑电图二者结合在癫病因诊断及定位诊断中起到相辅相成的作用,对癫的治疗、预后帮助很大。

CORRELATION STUDIES BETWEEN LONG-TERM MONITORING OF EEG AND CT EXAMINATION IN PATIENTS WITH EPILEPSY

Objectives:To explore the role of LTME and CT in the diagnosis and etiology of epilepsy.Methods:The results of 320 patients with epilepsy were analyzed by LTME(SOLAR ROVER-8 type AEEG and SOLAR 2000 type VEEG)and GE1800-CT from Jan.2005 to Dec.Results:LTME was normal in 123 cases(38.4%),abnormal was 197 cases(61.6%).48(15.0%)cases with clinical attackaccounted for 24.4% of the total abnormal EEG.The 114 cases with epileptic wave accounted for 57.9% of the total abnormal EEG,83(42.1%)caseswere atypical abnormal.There was no significant difference between clinical attacking rate and abnormal rate of AEEG and VEEG(x2=1.32,P>0.05;x2=0.93,P>0.05).CT was normal 229 cases(71.6%),abnormal 91 cases(28.4%).There was significant difference in the abnormal rate between CT group and LTME group(x2=10.88,P<0.001).Conclusions:The clinical using of LTME has greatly improved the positive rate of EEG of epilepsy,especially for suspicious patients without obvious clinical attacking and premonitory indications.CT has also greatly improved the positive rate of EEG of epilepsy caused by brain cause,decreased the diagnostic rate of primary epilepsy.As an important way,CT can find out the most important and possible cause of epilepsy.CT combining with EEG will play an important role in the diagnosis and treatment of epilepsy.
