云南省宁洱县农村老年人抑郁状况及其对生命质量的影响研究

目的分析云南省宁洱县农村老年人的抑郁状况及其对生命质量的影响。方法采用多阶段分层随机法抽取宁洱县1629名60岁及以上农村老年人进行现场面访,问卷内容包括基本信息、抑郁量表GDS-30和简明健康量表SF-36,采用GDS-30评价其抑郁程度,SF-36量表评估其生命质量得分。结果云南省宁洱县农村老年人的抑郁率为12.0%,其中男性为8.9%,女性为14.2%。女性、教育水平为文盲、家庭人均年收入低和医疗服务可及性差的老年人其抑郁率更高(P<0.05);非抑郁患者的生理功能(PF)、生理职能(RP)、躯体疼痛(BP)、总体健康状况(GH)、生命力(VT)、社会功能(SF)、情感职能(RE)、心理健康(MH)和躯体综合(PCS)、心理综合(MCS)的生命质量得分均高于抑郁患者(P<0.001);多元线性回归结果显示,女性、教育水平为文盲、抑郁的老年人生命质量PCS和MCS得分低(P<0.05),家庭人均年收入越差的老年人MCS得分越低(P<0.05)。结论抑郁降低了云南省宁洱县农村老年人的生命质量。应重点关注当地高危群体的心理健康,减少抑郁的发生,改善其生命质量。

Prevalence of Depression and Its Impact on Quality of Life among Rural Elderly People in Ninger County,Yunnan Province

Objective The aim of this study was to analyze prevalence of depression and its impact on quality of life among elderly people in rural Yunnan province.Methods A multi-stage stratified random method was used to select 1629 rural people aged 60 years and over in Ninger county.Each participant received a face-to-face interview.Questionnaires include basic information,Geriatric Depression Scale-30 and Concise Health Scale SF-36.GDS-30 was used to evaluate the degree of depression and SF-36 scale was used to assess quality of life score.Results The prevalence of depression among the participants was 12.0%,which was 8.9%for men and 14.2%for women.Women,older people with lower level of education,lower annual per capital family income and poor access to medical services had higher prevalence of depression than their counterparts(P<0.05).Non-depressive older people had lower scores in domains of physical functioning(PF),role physical(RP),bodily pain(BP),general health(GH),vitality(VT),social functioning(SF),role emotional(RE),mental health(MH),physical component summary(PCS),and mental component summary(MCS)than depressive older people(P<0.001).The results of multiple linear regression showed that female、low educational level and depressive elderly people tended to have lower scores in PCS and MCS domain(P<0.05).People with low annual household incomes tended to have lower scores in MCS domain(P<0.05).Conclusion Depression reduced the quality of life of rural elderly people in Ningler county,Yunnan province.Focus should be put on mental health status of local population with high risk to reduce the prevalence of depression and improve the quality of life.
