儿童促甲状腺激素地区性参考区间建立与临床应用

目的：建立河北省儿童促甲状腺激素（TSH）及甲状腺素（T3、T4、FT3、FT4）参考区间,并应用于临床筛查儿童甲状腺疾病。方法：选择黄骅市、保定市、张家口市健康儿童,测定血清中TSH、T3、T4、FT3、FT4,建立参考区间。分别将新建立的参考区间与试剂盒提供的参考区间作为实验室诊断标准,分析诊断三个地区800例儿童的甲状腺疾病发病情况。结果：建立的河北省儿童促甲状腺激素及甲状腺激素参考区间是：TSH 0.50-6.88μIU/ml,T30.52~2.36 ng/ml,T445.28~127.42 ng/ml,FT32.30~10.29 pmol/L、FT47.99-24.88 pmol/L。800例儿童采用该参考区间作为诊断标准,共检出45例异常结果;采用试剂盒提供的参考区间作为诊断标准,共检出120例异常。结论：该参考区间为河北省儿童促甲状腺激素提供了基础性数据,有利于儿童亚临床甲状腺减退的监测工作。

Establishment and Clinical Application of Regional Reference Range for Children Thyrotropin

Objective ： To establish the reference ranges of total triiodothyronine （TTa） ,total thyroxine （TT4）, free triiodothyronine （FT3）, free thyroxine （FT4）and Thyrotropin （TSH） of children,and use the results as criterion to screen children Thyroid diseases. Methods ：To select normal children from Huanghua City,Baoding City and Zhangjiakou City, and determinate the concentration of TSH ,Ta ,T4 ,FTaand FT4in serum so as to establish the reference ranges. Using the results and the reference ranges from reagent kit, to investigate the situation of 800 children Thyroid from Huanghua City, Baoding City and Zhangjiakou City. Result ..The newly established reference ranges were as follows ：TSH 2.74（0.50-6.88） μIU/mI,Ta 1.42（0. 52-2.36） ng/ml,T4 81. 17（45.28-127.42） ng/ml,FTa 6.47（2.30-10. 29） pmol/L,FT4 14.41（7.99-24.88） pmol/L. As the result to diagnostic standard of thyroid diseases,we had found out 45 persons ab normal finally. However ,as the reference ranges from the reagent kit to diagnostic standard of thyroid diseases ,it showed that 120 persons were abnormal. Conclusion：h provides basis for establishing the reference ranges of TSH in children. It should benefit for the monitor of children subclinical hypothyroidism disease.
