皮肌炎患者外周血单核细胞PD-L1分子的表达及其临床意义

目的:探讨皮肌炎患者外周血单核细胞PD-L1分子的表达水平及其与疾病活动的相关性。方法:收集25例皮肌炎患者及25例健康对照组外周血,Ficoll梯度分离外周血单核细胞,利用流式细胞仪检测PD-L1在CD14+单核细胞表面的表达,分析单核细胞表面PD-L1分子表达水平与炎症指标、典型皮损和肺间质病变等疾病活动指标的关系。结果:发现与健康对照组比较皮肌炎患者单核细胞上PD-L1分子高表达,ESR升高患者的外周血单核细胞PD-L1分子表达水平明显高于ESR正常患者,CRP升高患者的外周血单核细胞PD-L1分子表达水平明显高于CRP正常患者,具有Gottron征患者外周血单核细胞PD-L1分子表达水平明显高于无Gottron征患者,具有肺间质病变患者外周血单核细胞PD-L1分子表达水平明显高于无肺间质病变患者。结论:皮肌炎患者外周血单核细胞PD-L1分子与炎症指标、皮损和肺间质病变显著相关。

Expression and clinical significance of PD-L1 molecule on monocyte in peripheral blood of patients with dermatomyositis

Objective:To explore the expression of programmed death ligand 1(PD-L1)on CD14 monocyte in the patients with dermatomyositis(DM)and their disease activity.Methods:We collected blood samples of 25 DM patients and 25 healthy controls monocytes were isolated from peripheral blood with Ficol solution.The expression of PD-L1 and CD14 on the monocyte was examined by flow cytometry.Then,the association of expression of PD-L1 on CD14 monocyte with inflammatory biomarkers,skin lesions and severity of ILD was analyzed.Results:The expression of PD-L1 on CD14 monocyte was significantly increased in DM patients as compared with healthy controls.The DM patients with elevated erythrocyte sedimentation rate(ESR)showed significantly higher expression of PD-L1 compared with patients with normal ESR.The expression of PD-L1 in DM patients with raised Creactive protein(CRP)was significantly higher than those in patients with normal CRP.Moreover,the patients with Gottron′s sign showed significantly higher expression of PD-L1 compared with those without Gottron′s sign.The expression of PD-L1 was significantly higher in the patients with DM complicated by interstitial lung disease(ILD)than in those without ILD.Conclusion:The aberrations of expression of PD-L1 on CD14 monocyte were observed in the patients with DM.Expression of PD-L1 on CD14 monocyte was highly associated with inflammatory biomarkers,skin lesions and ILD in patients with DM.
