综合护理干预在负压封闭引流治疗骨折合并软组织缺损中的应用

目的探讨综合护理干预在负压封闭引流（VSD）治疗骨折合并软组织缺损中的应用效果。方法将住院治疗的135例骨折合并软组织缺损患者随机分为对照组67例，治疗组68例，2组创面均采用常规VSD治疗，治疗组在常规护理的基础上给予心理护理、健康教育及VSD护理等综合护理干预措施，将2组患者在治疗依从性、并发症的发生情况及VSD维持时间、敷料更换次数、创面闭合时间等方面进行统计比较。结果对照组患者治疗依从性为86．57％，VSD术后并发症发生率为40．30％，VSD敷料持续时间为（5．3±0．6）d，VSD敷料更换次数为（3．7±0．1）次，创面闭合时间为（21．3±4．3）d；治疗组患者治疗依从性为98．53％，VSD术后并发症发生率为4．41％，VSD敷料持续时间为（10．1±0．2）d，VSD敷料更换次数为（1．1±0．2）次，创面闭合时间为（12．5±2．7）d。2组之间比较，差异均有统计学意义（P〈0．05）。结论有效的综合护理干预可减少VSD术后并发症的发生，提高患者治疗的依从性，节约治疗成本，减少护士工作量，缩短创面闭合的时间。

Application of comprehensive nursing intervention in vacuum sealing drainage for fractures complicated with soft tissue defects

Objective ] To explore the effect of comprehensive nursing intervention in vacuum sealing drainage （VSD） for fractures complicated with soft tissue defects. Methods] 135 inpatients with fractures and soft tissue defects were randomly divided into the control group （67 cases） and the treatment group （68 cases）. The wound of all patients was treated with conventional VSD treatment. On the basis of conventional nursing care, the treatment group received the comprehensive nursing intervention measures, including mental nursing, health education and VSD nursing. The differences in treatment compliance, incidence rate of complications, VSD duration, frequency of changing dressings and wound closure time between two groups were statistically compared. Results] In the control group, the treatment compliance was 86.57%, the incidence rate of complications was 40.30%, VSD du- ration was （ 5.3 ＋ 0.6 ） days, frequency of changing dressings was （ 3.7 ＋ 0. 1 ） times, and the wound closure time was （21.3 ＋4.3） days. In the treatment group, the treatment compliance was 98.53% , the incidence rate of complications was 4. 41%, VSD duration was （ 10.1 ±0.2） days, frequency of changing dressings was （ 1.1 ±0.2） times, and the wound closure time was （ 12.5 ± 2.7 ） days. The differences between two groups were statistically significant （ P 〈 0.05 ）. Conclusion ] The effective comprehensive nursing intervention can reduce the incidence of postoperative complications of VSD, improve patients＇treatment compliance, save the medical cost, reduce the workload of nurses, and shorten the time of wound closure.
