黄葵胶囊对原发性痛经的临床防治及其机制研究

目的观察黄葵胶囊对原发性痛经的临床疗效及机制。方法将2012年6月—2013年4月天门市第一人民医院96例原发性痛经患者纳入研究，并随机分为2组：对照组（n=38）与干预组（n=58）。所有患者均给予营养支持、心理疏导及镇痛药物等常规治疗。干预组在上述治疗的基础上加用黄葵胶囊治疗。采用总的痛经评分（TDS）和视觉模拟评分法（VAS）评价药物疗效。同时测定患者黄体末期血清雌孕激素浓度，随访观察药物不良反应。结果干预组总有效率（96．5％）高于对照组总有效率（84．2％）（P〈0．05）；经过治疗后，2组患者TDS评分和VAS评分均明显下降（P〈0．01），干预组更为明显（P〈0．01）；治疗后，相比对照组，干预组患者血清雌激素水平明显下降，而孕激素水平显著上升（P〈0．01）。结论黄葵胶囊对原发性痛经有较好的防治作用，且安全无毒，其作用可能与调节雌孕激素有关。

Study on clinical efficacy and mechanism of Huangkui Capsule on primary dysmenorrheal

Objective] To observe the clinical efficacy and mechanism of Huangkui Capsule on primary dysmenorrhea （PD）. Methods] 96 PD patients. who were treated in the First Peoples Hospital of Tianmen City from June 2012 to April 2013 , were enrolled in this study and were divided into 2 groups： the control group （38 cases） and the intervention group （58 cases）. All patients received the conventional treatment. including nutrition support. psychological counseling. and analgesic treatment. The intervention group was given Huangkui Capsule on the basis of conventional treatment. The clinical efficacy of Huangkui Capsule was evaluated by the total dysmenorrhea score （TDS） and visual analog scale （V AS）. The levels of serum estrogen and progesterone in the final luteum phase were measured. and the adverse reactions were followed up. Results] The total effective rate of the intervention group （96.5% ） was higher than that of the control group （84.2%） significantly （P 〈0. 05）. After treatment. the scores of TDS and V AS reduced significantly in both groups （P 〈 o. 01 ） ,especially in the intervention group （P 〈 0. 01 ）. Compared to the control group, the level of serum estrogen in the intervention group decreased significantly. while the level of serum progesterone increased significantly （P 〈 0. 01 ） . Conclusion] Huangkui Capsule could significantly alleviate primary dysmenorrhea. which is safe without adverse reactions. The mechanism might be related to the changes of serum estrogen and progesterone.
