胰十二指肠切除术快速康复外科监护

目的探讨快速康复外科监护对胰十二指肠切除术围手术期的效果。方法选自2014年4月至2015年7月300例施行胰十二指肠切除术的患者,围手术期随机分为两组,即以传统监护的传统组和快速康复外科监护的实施组各150例。数据采用软件SPSS 16.0进行处理,术后并发症、满意度调查计数资料采用χ~2检验,P0.05则认为差异具有统计学意义。结果 300例患者胰十二指肠切除术均顺利完成,其中实施组患者术后并发症总例数5例(3.3%);传统组中术后并发症总例数13例(8.6%),实施组术后并发症发生率明显低于传统组,差异有统计学意义(χ~2=4.551,P=0.033)。同时患者满意度调查显示,实施组满意率98.0%(147/150),传统组满意率93.3%(140/150),实施组满意度高于传统组,差异有统计学意义(χ~2=3.940,P=0.047)。结论胰十二指肠切除术的快速康复外科监护,可降低手术并发症的发生,提高患者术后满意度,改善患者生活质量。

Perioperative application of Fast Track Rehabilitation in Pancreaticoduodenectomy

Objective To investigate perioperative application of fast track rehabilitation in patients underwent pancreaticoduodenectomy.Methods From April 2014 to July 2015,300 patients received pancreaticoduodenectomy were randomly divided into experimental group (n =150) and traditional group (n=150).Clinical data were analyzed retrospectively,including postoperative complications and postoperative satisfaction.Statistical analysis were performed by using SPSS (version 16,SPSS Inc,Chicago,IL).The postoperative complications,satisfaction survey count data using x2 test.A P value ＜0.05 was considered as statistically significant difference.Results All of 300 patients underwent pancreaticoduodenectomy successfully.In experimental group,there were 5 cases (3.3％) of postoperative complications with postoperative satisfaction of 98.0％ (147/150).In traditional group,there were 13 cases (8.6％) of postoperative complications with postoperative satisfaction of 93.3％ (140/150).There were significant difference between 2 groups in terms of postoperative complications and postoperative satisfaction rate respectively (t2 =4.551,P =0.033;x2 =3.940,P =0.047).Conclusion For patients underwent pancreaticoduodenectomy,the fast track rehabilitation could reduce incidence of surgical complications and improve patient's satisfaction and the quality of life.
