脑外伤去骨瓣减压术后并发症临床分析（附87例报告）

目的探讨重型颅脑外伤去骨瓣减压术后并发症发生的原因及防治措施。方法回顾分析2006年。2009年我科87例闭合性颅脑外伤去骨瓣减压术后发生的并发症及其发生率、类型与危险因素。结果术后3个月临床随访结果：GOS5分54例，4分21例，3分7例，2分5例；去大骨瓣术后出现并发症以脑膨出、迟发性颅内出血、硬膜下积液多见。结论去骨瓣减压术可能因为发生并发症而降低效果，各种类型并发症有其典型时间窗口发生，对去骨瓣减压术后并发症积极防治、正确处理能有效降低术后死亡率，改善预后。

Clinical analysis of complications after decompressive craniectomy for traumatic brain injury

Objective To study the causes of post-operative complications of decompressive cranieetomy for severe traumatic brain injury ,as well as the approaches to its prevention and treatment.Methods A clinical data of the postoperative complications for 87 patients with close traumatic brain injury in our department who underwent decompressive craniectomy between 2006 and 2009 were analyzed retrospectively.Results Patients with Glasgow outcome scale at postoperative 3 months follow up.54 had a Glasgow outcome score of 5,21 had a glasgow outcome score of 4, and 7 had a glasgow outcome score of 3,5 had a glasgow outcome score of 2 .Among Postoperative complications,brain swelling through craniectomy defect,delayed intracranial hemorrhage and subdural effusion were more common. Conclusion The potential benefits of decompressive craniectomy can be adversely affected by the occurrence of complications. Each complication secondary to large decompressive craniectomy had its own typical time window for occurrence. The active prevention and proper treatment of complications, can effectively decrease the postoperative mortality and improve the prognosis.
