心理干预对严重烧伤患者心理健康水平和应对方式的影响

目的：探讨心理干预对严重烧伤患者心理健康、社会支持状况和应对方式的影响。方法将84例严重烧伤患者随机分为两组，研究组43例，对照组41例，两组均接受烧伤科常规治疗及护理，研究组在此基础上联合心理干预，观察12周。于心理干预前后采用症状自评量表、社会支持评定量表及应对方式问卷进行测评分析。结果治疗12周末，研究组症状自评量表总分及9个因子分和对照组躯体化、人际关系敏感、抑郁、恐怖因子分较入组时基线分显著降低（P＜0．05或0．01），且研究组总分、躯体化、强迫、焦虑及其他因子分均显著低于对照组（ P＜0．05或0．01）；研究组社会支持评定量表主观支持、支持利用度因子分和应对方式问卷积极应对维度分均较入组时基线分显著提高（ P＜0．05或0．01），对照组主观支持因子分和积极应对维度分也较入组时基线分显著提高（ P＜0．05或0．01），但研究组显著高于对照组（ P＜0．05或0．01）。结论心理干预能有效缓解严重烧伤患者的负性心理，提高患者的社会支持度和积极应对方式，对提高患者的心理健康水平具有重要作用。

Influences of psychological intervention on mental health levels and coping styles of patients with severe burn

Objective To explore the influences of psychological intervention on mental health levels ,social support status and coping styles of patients with severe burn .Methods Eight-y-four severe burn patients were randomly divided into research group （n=43） and control group （n=41） , both groups received routine treatment and nursing of burn department ,research group was plus psycho-logical intervention for 12 weeks .Assessments were conducted with the Symptom Checklist-90 （SCL-90） , Social Support Rating Scale （SSRS） and Coping Style Questionnaire （CSQ） before and after intervention . Results At the end of the 12th week the total and 9 factor score of the SCL-90 in research and somatiza-tion ,interpersonal relation sensitivity ,depression and phobia in control group lowered more significantly compared with pre-intervention （P〈0 .05 or 0 .01） ,the total ,somatization ,obsession ,anxiety and other factor score were all significantly lower in research than control group （P〈0 .05 or 0 .01）;subjective sup-port and support availability of the SSRS and active coping of the CSQ in research group elevated more sig-nificantly compared with pre-intervention （P〈0 .05 or 0 .01） ,so did the subjective support and active cop-ing in control group （P〈0 .05 or 0 .01） ,but those were significantly higher in research than control group （P〈0 .05 or 0 .01） .Conclusion Psychological intervention could effectively relieve negative psychology of patients with severe burn ,it is important for improving patients’ mental health level to increase patients’ social support and active coping styles .
