磁共振成像T1值、T2值和ADC值与肝脏纤维化模型病理评分分级纤维化分期诊断结果相关性研究

摘    要:目的:探讨磁共振成像T1值、T2值、ADC值对肝脏纤维化病理评分分级纤维化分期诊断的价值和意义。方法:将大白兔分为正常对照组(4只)和肝脏纤维化模型组(13只),分别对其进行常规MR扫描,测量肝脏组织T1值和T2值;磁共振弥散加权成像扫描(MR-DWI),测量肝脏组织表观扩散系数(ADC值)。取肝脏组织标本做常规HE染色和网状纤维染色(Gomoris法),以病理组织学评分分级和纤维化分期诊断结果为标准,研究分析T1值、T2值和ADC值在肝脏纤维化不同评分分级和纤维化分期间的变化情况;采用统计学相关分析法研究T1值、T2值和ADC值在与病理积分、评分分级和纤维化分期诊断结果之问的相关性。统计学分析采用SPSS13.0,单相分析法,P〈0.05为差异有统计学意义。结果:磁共振成像T1值、T2值、ADC值测量结果在对照组和不同评分分级和纤维化分期间的差异性具有显著的统计学意义(P〈0.05);在肝脏纤维化模型组中T1值和T2值明显高于对照组,且随着评分分级和纤维化分期的增加而逐渐增加;肝脏纤维化模型组中ADC值明显低于对照组,随着评分分级和纤维化分期的增加降低越显著。相关分析结果表明T2值与病理积分、评分分级和纤维化分期诊断结果之间呈显著正相关性(P〈0.01);ADC值与病理积分、评分分级和纤维化分期诊断结果之间表现显著负相关性(P〈0.01)。结论:在肝脏发生纤维化时,定量检测磁共振成像中的T1值、T2值和ADC值,能够评估肝脏纤维化病理评分分级和纤维化分期,作为非损伤性检查方法的磁共振成像可以用于临床肝脏纤维化定量诊断。

Abstract:Objective:To study the values of MRI T1-value, T2-value and ADC-value in diagnosis for pathological scores grading and fibrosis staging of liver fibrosis. Methods: Four normal white rabbits and thirteen wtfite rabbits of liver fibrosis model were been examined by conventional magnetie resonance imaging( MIH) and magnetic resonance diffusion weighted imaging (MR-DWI), The T1-value and T2- value of liver tissue were measured on MRI and the apparent dittasion coefficient (ADC-value) of liver tissue were measured on MR-DWI. The all liver specimens were made with the stains of conventional H&E as well as for fibrosis (Gomori' s stain), and the diagnosis of histopafliulogie scored grading and fibrosis staging were made. The study mid analysis were made for the change of the T1-Value, T2-value arid ADC-value in different grading arid fibrosis staging as well as file eoIrelation with patliological seores, grading arid staging according to the lfistopatliologie criteria. The statistical package of SPSS 13.0 and one-way analysis of variance (analysis of variance, ANOVA) were used. P 〈 0.05 indicated that the difference was statistically fignificant. Results: It was great statistiea/significance that the difference of Tl-value, T2-value and ADC-value of MRI in the normal control group and different grading arid fibrosis staging( P 〈 0.05). The T1- value and T2-value in experimental group were higher titan those in the control group. The ADC-values in experimental group were sigrfifi- eanfly lower than those in the control group. Using statistical correlation analysis methods, it was been showed that the T2-value was sigififieant positive correlation arid ADC-values was significant negative correlation with pathological scores, grading and staging( P 〈 0.01 ). Concluslon:When liver fibrosis occurred, the histopathologie scored grading and fibrosis staging could be evaluated by quantitative measured of Tl-value, T2-value and ADC-value in MRI.
