改良拉玛泽减痛分娩法对分娩结局影响的研究

目的：观察改良拉玛泽减痛分娩法对分娩结局的影响,探讨其临床应用价值.方法：观察组300例初产妇自孕28周开始,给予改良拉玛泽减痛分娩法培训,临产后实施改良拉玛泽减痛分娩法;对照组300例初产妇未参加培训,行常规产科护理;观察两组产妇分娩过程中的镇痛效果、产程、分娩方式及并发症.结果：与对照组比较,观察组产妇疼痛程度减轻、自然分娩率升高、产程时间缩短、产后2h及24 h出血量减少、新生儿窒息率降低,两组比较差异均有统计学意义（P＜0.05）.结论：改良拉玛泽减痛分娩法能安全有效地减轻分娩过程中的疼痛,增加自然分娩率,降低剖宫产率,缩短产程,减少产后出血量及新生儿窒息率.

STUDY ON THE EFFECTS OF MODIFIED LAMAZE CHILDBIRTH METHOD ON OUTCOMES OF LABOR AND DELIVERY

Objective：To observe the effects of modified Lamaze less painful childbirth method on delivery process and delivery outcomes,and explore its clinical value.Methods：A total of 300 primiparas in obser vation group were trained by modified Lamaze less painful childbirth method from the beginning of 28 gestational weeks,and implemented it from the onset of labor.A total of 300 primiparas in control group were only provided with the regular obstetric care,without being trained of modified Lamaze less painful childbirth method.Then observe the analgesic effect,labor,mode of delivery and complication between the two groups.Results：Compared to the control group,the observation group showed significant decrease in delivery pain,increase in natural childbirth rate,shortening of labor time,reduction of blood loss during the first 2 hours and 24 hours after delivery,and decrease in neonatal asphyxia rate （P 〈0.05）.Conclusion：Modified Lamaze less painful childbirth method can effectively and safely reduce pain in the delivery process,increase natural childbirth rate,shorten the stage of labor,and reduce the cesarean section rates,the postnatal blood loss,and the neonatal asphyxia rates.
