MCL-1蛋白在B细胞非霍奇金淋巴瘤的表达及临床意义

目的:分析髓细胞白血病1(MCL-1)蛋白在B细胞非霍奇金淋瘤(B-NHL)的表达,探讨其与细胞凋亡和增殖的相互关系及其潜在的临床意义。方法:用免疫组织化学SP法检测43例不同类型的B-NHL的淋巴结组织以及结外肿瘤组织和10例反应增生淋巴结(RH)中的MCL-1蛋白表达。结果:43例不同类型的B-NHL的MCL-1蛋白的表达中,弥漫大B细胞淋巴瘤的阳性率最高为70.04%(19/27),侵袭性B-NHL表达的频率和强度明显高于惰性B-NHL70.00%vs30.76%,P<0.05,而10例RH的MCL-1蛋白无阳性表达。MCL-1蛋白的表达与B细胞NHL的亚型密切相关(P<0.001),与临床分期无明显相关,MCL-1蛋白的表达阳性组与细胞增殖指数(PI)呈正相关(P=0.002),与凋亡指数(AI)无相关。结论:MCL-1蛋白在侵袭性B-NHL易于表达,其表达与B-NHL亚型相关,并与淋巴瘤细胞的侵袭性和增殖状态有关,推测MCL-1蛋白在B-NHL发生发展中具有潜在作用。

Expression and Clinical Significance of MCL-1 in B-Cell Non-HodgKin's Lymphoma

Objective: To investigate the expression of MCL-1 in B-cell non-Hodgkin's lymphoma and its clinical significance.Methods: Immunohistochemical assay was used to detect the expression of MCL-1 in 43 cases of B-cell NHL and 10 cases of reactive hyperplasia lymph node.Results: By using immunohistochemistry,a subset of neoplasms in 6 lymphoma types studied expressed MCL-1,the positive expression of MCL-1 in diffuse large B-cell lymphoma was 70.04%(19/27,but expression was more frequent and intense in aggressive B-NHL(21 of 30,70.00 %) compared with indolent B-NHL(4 of 13,30.76 %) lymphoma(P< 0.05).Whereas no expression of MCL-1 was detected in the 10 case of RH lymph node.The expression of MCL-1 was statistically significant associated with the subset of B-cell NHL,rather than the clinic period.Proliferate index(PI) correlated positively with expression of MCL-1(Z=-3.053,P=0.002),but the expression was not correlated with apoptotic index(AI).Conclusion: Our data suggest that MCL-1 is frequently expressed in aggressive B-cell NHL,MCL-1 expression also correlates with grade and aggressive and proliferation state.MCL-1 may contribute to transformation and development in B-cell NHL.
