重型和普通型严重传染性非典型肺炎患者淋巴细胞亚群改变初步分析

摘    要:目的 研究传染性非典型肺炎(SARS)重型和普通型患者外周血淋巴细胞亚群异常变化,以期完善诊断标准。方法 采用多色荧光标记流式细胞术结合血液分析仪检测155例SARS患者的外周血淋巴细胞及其亚群的表达情况。结果 普通型组与重型组比较,外周血白细胞、淋巴细胞、CD3 CD4 、CD3 CD4-CD8-、NK、CDl9 CD5 、CD45RA CD4 、CD45RO CD4 、CD28 CD4 、CD28 CD8 、CD28 CD8-、CD95 CD4 等亚群的绝对值差异有显著性,两组均低于健康人,且重型组低于普通型组。结论 SARS患者的大部分淋巴细胞及亚群均受到破坏,以T淋巴细胞功能减低最为显著。外周血淋巴细胞各亚群的绝对值和百分比对于反映SARS患者免疫功能的改变情况均有意义。外周血淋巴细胞各亚群检测有望成为新的SARS实验诊断与监测指标。

Abstract:Objective To study changes of lymphocyte subsets in sev ere acute respiratory syndrome (SARS) patients including critical type and common type in order to increase diagnosis and establish a guide for therapy.Method Flow cytometer with multi-color fluroscence and hematology analyzer were used to detecte the expression of lymphocytes and its subsets in 155 cases of SARS including 25 cases of critical type and 130 cases of common type. Results Statistical analyse by means of SAS software show that there was significant decrease in counts of common type group on WBC, lymphocyte and some of its subsets such as CD3+,CD3+CD4+,CD3+CD4-CD8-,NK,CD19+CD5+,CD45RA+CD4+,CD45RO+CD4+,CD28+CD4+,CD28+CD8+,CD28+CD8- and CD95+CD4+ subsets when compared with critical type group. In addition, the subsets of critical type group and common type group less than healthy people, and critical type group decreased more markedly.Conclusion The lymphocytes and its subsets were impaired in SARS patients, especially for T cells and its subpopulations. Both absolute counts (AC) and percentages (PC) of subpopulation cells showed significant changes of immune cells in SARS, and AC seems to be more meaningful than PC. The detection of lymphocyte subsets are to be a new indicator for laboratory diagnosis and monitoring.
