醒脑静联合无创正压机械通气治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭的观察

目的探讨醒脑静联合无创正压机械通气(NIPPV)治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭的临床疗效。方法63例COPD合并Ⅱ型呼吸衰竭患者随机分为治疗组(31例)和对照组(32例)。2组均予抗感染、解痉平喘、肾上腺皮质激素及NIPPV等治疗;治疗组另加用醒脑静治疗。观察治疗后血气指标和临床症状的变化。结果治疗组总有效率(93.5%)高于对照组(71.9%),差异有统计学意义(P<0.05);2组治疗后PaO2、PaCO2、pH均改善(P<0.05,P<0.01),但治疗组优于对照组(P<0.05,P<0.01);治疗组患者神志改善时间(76min±48min)明显短于对照组(125min±77min),差异有统计学意义(P<0.05)。结论醒脑静联合NIPPV治疗COPD合并Ⅱ型呼吸衰竭具有良好效果。

Therapeutic effect of Xingnaojing injection combined with non-invasive positive airway pressure mechanical ventilation on chronic obstructive pulmonary disease complicated with type Ⅱ respiratory failure

Objective To observe the therapeutic effect of Xingnaojing injection combined with non-invasive positive airway pressure mechanical ventilation （NIPPV） on chronic obstructive pulmonary disease （COPD） complica ted with type Ⅱ respiratory failure. Methods 63 patients with COPD complicated with respiratory failure were randomly divided into two groups,treatment group（ n = 31 ）and the control group（ n = 32）. The patients in two groups were treated by inhaling oxygen, anti-infection, intravenous aminophylline and NIPPV, besides these therapy the patients in treatment group were treated by Xingnaojing injection. The arterial blood gas parameters and the changes of clinical symptoms were observed before and after treatment in both groups. Results The total effective rate（93.5% ） in treatment group was significantly higher than that（71.9% ） in control group（ P 〈 0.05）. There were significant improvements in PaO2, PaCO2 and pH in treatment group and in control group （ P 〈 0.01 or P 〈 0.05）, and treatment group was superior to control group（ P 〈 0.05, P 〈 0.01） .The improvement time （76 min ±48 min） of patients＇ mentality in treatment group was significantly shorter than that （ 125 min ± 77 min） of control group（ P 〈 0.05）. Conclusion Xingnaojing injection combined with NIPPV tins obvious therapeutic effect on chronic obstructive pulmonary disease complicated with type Ⅱrespiratory failure.
