摘    要:目的研究本地区血同型半胱氨酸水平与脑梗死及原发性高血压病的关系。方法收集脑梗死合并高血压病患者60例,原发性高血压病患者30例,正常对照组30例,测定血Hcy等指标,对脑梗死患者行NIH评分(NIH Stroke Scale)。结果 (1)脑梗死合并高血压病组、原发性高血压组及正常对照组血Hcy浓度分别为(27.16±10.46)μmol/L、(20.11±3.17)μmol/L、(13.35±1.82)μmol/L,组间比较差异有统计学意义(P<0.05)。(2)血Hcy水平与血糖、甘油三酯、低密度脂蛋白、血压的相关性分析表明血Hcy水平与各指标间无相关性,(P>0.05)。(3)脑梗死合并高血压组中高Hcy血症组的NIH评分明显高于非高Hcy血症组(6.97±3.76)分vs(2.82±1.01)分,P<0.01],而血压水平差异无统计学意义。结论高Hcy血症是脑梗死的独立危险因素,临床对于合并高血压等传统危险因素的脑梗死患者应积极筛查高Hcy血症。

Abstract:Objective To investigate the correlation between plasma homocysteine(Hcy) level and cerebral infarction and primary hypertension.Methods Sixty patients with cerebral infarction and hypertension were enrolled as group A,thirty patients with primary hypertension were selected as group B,and 30 healthy individuals were selected as the control group.The plasma levels of Hcy was detected in the three groups,and the traditional risk factors of cerebral infarction were recorded.Severity of cerebral infarction was assessed by NIH Stroke Scale(NIHSS).Results Plasma levels of Hcy in group A,group B and the control group were(27.16±10.46) μmol/L,(20.11±3.17) μmol/L,and(13.35±1.82) μmol/L,respectively,with statistically significant difference between the three groups.Plasma levels of Hcy was found to have no significant correlation with the levels of blood glucose,triglyceride,LDL,blood pressure in group A.Patients in group A was divided into hyper homocysteine(Hcy) group and non HHcy group.NIHSS of Hcy group(6.97±3.76) was significantly higher than that of non Hcy group(2.82±1.01).However no statistically significant difference in blood pressure was found between the two groups.Conclusion Hyper homocysteine was an independent risk factor for cerebral infarction.Patients with cerebral infarction should be detected for plasma Hcy levels.
