经颅多普勒超声在高血压人群体检中的应用价值

摘    要:目的探讨经颅多普勒超声(TCD)在高血压人群体检中的应用价值。方法选取2012年2月至2013年3月期间在健康体检中发现的高血压患者200例作为高血压组,另选同期健康体检且除外高血压病的正常人群150例作为对照组。所有受试人员均采用经颅多普勒超声诊断仪对颈内动脉系统及椎基底动脉系统的血管情况进行探查。比较分析两组受试者TCD检测结果、双侧大脑中动脉(MCA)及基底动脉(BA)各血液运动参数。结果与对照组健康受试者相比,高血压组患者TCD检测结果脑动脉硬化频谱改变比率、血流速度升高比率以及血流速度降低比率均明显提高,正常比率明显下降,差异均具有统计学意义(P〈0.05)。与此同时,与对照组健康受试者相比,高血压组患者双侧MCA及BA血液运动参数平均血流速度、峰值流速及舒张期流速有所降低,而脉动指数和搏动指数有所升高,差异具有统计学意义(P〈0.05)。结论经颅多普勒超声能够确切地反映脑部血流动力学的状况以及基本的病理改变,有助于高血压病情的评估及诊断,为临床治疗合理方案的选择提供了参考。

Abstract:Objective To probe into the application value of transcranial Doppler ultrasound (TCD) examination for hypertension in health examination. Methods Two hundred hypertension patients diagnosed in health examination conducted from February 2012 to March 2013 in our hospital were selected as hypertension group. And 150 normal people participated in the health examination during the same period as control group. The internal carotid artery and vertebrobasilar system were examined with transcranial Doppler ultrasonography in all subjects. TCD examination results, blood flow parameters of bilateral middle cerebral artery (MCA) and basilar artery (BA) in two groups were comparatively analyzed. Results Compared with the control group, the rate of cerebral arteriosclerosis, the rate of increased blood flow velocity and the rate of reduced blood flow velocity increased significantly, while the normal rate decreased significantly in hypertension group ( all P 〈 0.05 ). At the same time, compared with the control group, the blood mean flow velocity, peak velocity and end - diastolic velocity of bilateral MCA and BA decreased significantly, the pulsatility index increased significantly in hypertension group ( all P 〈 0.05 ). Conclusion TCD may reflect brain hemodynamic status and basic pathological changes exactly and contribute to the assessment and diagnosis of hypertensive disease. The application of TCD provides a reference for a reasonable solution for the clinical treatment of choice.
