腹腔镜联合术中胆道镜治疗胆总管结石18例临床分析

目的:探讨腹腔镜联合胆道镜治疗胆总管结石的应用体会.方法:回顾性分析我院2008年10月-2010年10月收治的腹腔镜联合胆道镜行胆总管探查取石术18例的临床资料.结果:16例腹腔镜下完成手术,2例术中转开腹.术后胆漏1例,经引流痊愈;5例胆总管残余结石,术后行ERCP或经T管窦道胆道镜取石;平均住院时间(7.0±3.2)d.结论:腹腔镜联合胆道镜在胆总管切开取石术中的应用具有疗效好、并发症少、安全等优点.

Clinical Analysis of 18 Patients with Choledocholithiasis Treated by Laparoscope and Intraoperative Choledochoscopy

Objective： To explore the operative experiences of laparoscope and intraoperative choledochoscopy for patients with choledoeholithiasis. Methods：The clinical data of 18 patients with choledocholithiasis treated by laparoscope and intraoperative choledochoscopy from Oct 2008 to Oct 2010 were analyzed retrospectively.Results： Among the 18 cases, 16 eases were successfully completed the laparoseope,and 2 cases were transferred to laparotomy during the laparoscopie procedure. 1 case was found postoperative bile leakages and were cured by drainage. 5 cases of residual common bile duct stones were cured by the postoperative endoseopieretrograde cholangiopancretography or trans-T tube eholangioseopy 4 weeks after operation. The average hospital stay was （7.0±3.2） days.Conclusion： Laparoscopic and choledochoscopic common bile duct exploration is of good efficacy, few complications and safe treatment for patients with cholecystolithiasis.
