3.0T磁共振扩散加权成像诊断胰腺癌

摘    要:目的 探讨3.0T MR扩散加权成像及其表观扩散系数(ADC值)对胰腺癌的诊断价值.方法 对经手术病理证实的胰腺癌患者30例及对照组30例行DWI检查(b值=800 s/mm~2),测量肿瘤组织、瘤周组织及对照组正常胰腺组织的 ADC值,并对所得结果进行统计学检验.结果 ①肿瘤组织、瘤周组织和正常胰腺组织的平均ADC值分别为(1.494±0.273)×10~(-3) mm~2/s、(1.631±0.281)×10~(-3) mm~2/s和(1.778±0.237)×10~(-3) mm~2/s.单因素方差分析显示肿瘤组织与正常胰腺组织间差异有统计学意义,而肿瘤组织与瘤周组织间平均ADC值差异无统计学意义.②以肿瘤组织平均ADC值95%可信区间的上限(1.622×10~(-3) mm~2/s)作为诊断胰腺癌的上限阈值点,诊断的灵敏度、特异度、准确率、阳性预测值、阴性预测值分别为73.33%、86.67%、80.00%、84.62%、76.47%,ROC曲线下面积Az=0.800.结论 胰腺癌组织与正常胰腺组织间平均ADC值差异具有统计学意义,测量ADC值对于诊断胰腺癌具有一定帮助.
