糖尿病合并慢性心力衰竭患者检测糖化血红蛋白和BNP的临床意义探讨

摘    要:目的探讨糖化血红蛋白和脑钠肽(BNP)检测对糖尿病合并慢性心力衰竭患者的临床意义。方法选取60例糖尿病合并慢性心力衰竭患者作为观察组,另选取同期60例糖尿病未合并慢性心力衰竭患者作为对照组。比较两组患者的糖化血红蛋白及BNP水平;观察组不同心功能分级患者的糖化血红蛋白及BNP水平;随访1年,比较两组患者的心脏不良事件发生情况。结果观察组患者的糖化血红蛋白为(10.3±2.3)%、BNP为(937.1±10.9)pg/ml,均明显高于对照组的(8.1±2.2)%、(190.3±8.4)pg/ml,差异均具有统计学意义(P<0.05)。观察组不同心功能分级患者的糖化血红蛋白及BNP水平比较,差异均具有统计学意义(P<0.05);随着心功能分级的增加,糖化血红蛋白及BNP水平增高。观察组患者的心脏不良事件发生率为41.7%,高于对照组的20.0%,差异具有统计学意义(P<0.05)。结论对糖尿病合并慢性心力衰竭患者监测糖化血红蛋白及BNP,可对患者病情及预后有较好的评估,对预防心脏不良事件的发生有重要作用。

Abstract:Objective To discuss the clinical significance of detection of glycosylated hemoglobin and brain natriuretic peptide(BNP)in patients with diabetes mellitus complicated with chronic heart failure.Methods There were 60 patients with diabetes mellitus complicated with chronic heart failure selected as the observation group,and 60 diabetes mellitus patients without chronic heart failure during the same period as the control group.Comparison was made on levels of glycosylated hemoglobin and BNP,and occurrence of adverse cardiac events after 1 year of follow-up between the two groups,levels of glycosylated hemoglobin and BNP in patients with different cardiac function grades in observation group.Results The glycosylated hemoglobin and BNP of the observation group were(10.3±2.3)%and(937.1±10.9)pg/ml,which were obviously higher than(8.1±2.2)%and(190.3±8.4)pg/ml of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant in levels of glycosylated hemoglobin and BNP in patients with different cardiac function grades in the observation group(P<0.05).With the increase of cardiac function classification,the levels of glycosylated hemoglobin and BNP increased.The incidence of adverse cardiac events of the observation group was 41.7%,which was higher than 20.0%of the control group,and the difference was statistically significant(P<0.05).Conclusion Monitoring glycosylated hemoglobin and BNP in patients with diabetes and chronic heart failure can provide a better assessment of the patient's condition and prognosis,and play an important role in preventing adverse cardiac events.
