宫颈薄层液基细胞学15000例检查结果及诊断分析

目的 探讨宫颈薄层液基细胞学(TCT)检查诊断及其在临床中的应用价值.方法 回顾性分析2013年2月至2015年5月在延安大学附属医院就诊的15 000例患者行TCT检查的资料,并对检查结果进行分析,其细胞学诊断标准采用TBS分类系统.结果 在15 000例患者中,炎症标本13 850例,占92.33%;意义不明的非典型鳞状上皮细胞(ASCUS)及以上级别标本860例,占5.73%;在TCT检查阳性病例中,有669例阴道镜下宫颈活检病理组织学诊断对照,其中活检阳性病例为393例,367例为鳞状上皮内病变,25例为鳞状细胞癌,1例为腺癌.细胞学阳性与活检病理组织学阳性的符合率为58.74%.结论 TCT仍是目前宫颈癌筛查的重要首选方法,诊断准确率高,可继续在临床推广应用.

Thinprep cytologic test results of 15 000 cases and diagnosis analysis

Objective To investigate the value of thinprep cytologic test (TCT) in diagnosis of cervical carcinoma.Methods Retrospective analysis was conducted on the data of 15 000 cases accepting TCT from February 2013 to May 2015 in Affiliated Hospital of Yan'an University, and the results were analyzed.TBS classification system was taken for cytological diagnosis.Results In 15 000 patients, 13 850 cases were inflammatory, accounting for 92.33%.There were 860 cases of undetermined significance of atypical squamous cells (ASCUS) and above level, accounting for 5.73%.Among TCT positive cases, 393 of 669 cases undertaking control colposcopic cervical biopsy were positive, including 367 cases of squamous intraepithelial lesions, 25 cases of squamous cell carcinoma, and 1 case of adenocarcinoma.The coincidence rate of positive biopsy pathologic histology and positive cytology was 58.74%.Conclusion TCT is still the most important method of cervical cancer screening with high diagnostic accuracy, and it can continue to be used in clinical practice.
