人工全髋关节置换术后外侧小切口与标准切口治疗老年股骨颈骨折的临床研究

目的探讨行人工全髋关节置换术后外侧小切口与后外侧标准切口治疗老年股骨颈移位骨折的临床疗效。方法选取2008年7月至2011年6月本院骨外科收治的老年股骨颈骨折患者共88例,随机分为两组,观察组采用后外侧小切口人工全髋关节置换术治疗;对照组行后外侧标准切口人工股骨头置换术治疗。分别比较两种术式的术中失血量、术后恢复情况以及关节功能改良率。结果两组在手术时间、平均出血量、术后下床活动时间方面比较,差异均有统计学意义(P<0.01);两组髋关节功能优良率比较,观察组优于对照组(P<0.05)。结论两种术式均为治疗老年股骨颈骨折患者的有效治疗方法,后外侧小切口术比较安全,且近期疗效优于后外侧标准切口人工全髋关节置换术。

Clinical Study of Posterolateral Small Incision and Standard Incision for Total Artificial Hip Arthroplasty Treating Femoral Neck Fracture in the Elderly

Objective To discuss the clinical effect of posterolateral small incision and standard incision for total artificial hip arthroplasty treating femoral neck displaced fracture in the elderly.Methods A total of 88 cases of elderly femoral neck fracture admitted to the Bone Surgery Department of our hospital during July 2008-June 2011 were selected and randomly divided into two groups,observation group adopted small posterolateral incision for total hip arthroplasty in the treatment;control groups adopted the standard posterolateral incision for the surgery.Blood loss during surgery,postoperative recovery and joint function improvement rate of the two groups were compared respectively.Results The operation time,the average blood loss,postoperative ambulation time of the two groups were statistically significantly different(P<0.01);hip function excellent rates of observation group was higher than the control group(P<0.05).Conclusion Two kinds of operation are both effective treatments of femoral neck fractures in the elderly patients,while posterolateral small incision surgery is safer and has short-term effect than the standard posterolateral incision for total hip arthroplasty.
