基层医院门诊退药原因分析及对策

目的:寻找符合我院实际的管理措施,提高合理用药水平。方法:收集2010年8月23日-2010年11月3日我院门诊西药房退药申请单180张,涉及药品367种,以科室为标杆,按退药原因、退药品种、贮存条件进行分析。结果:退药科室主要分布在儿科,退药主要原因有不良反应/事件(ADR/ADE)、病情变化、不合理用药等,分别占退药次数的41.2%、26.3%、19.3%,累计达86.8%。所退药品主要为常温贮存下的西药。结论:应以确保药品质量为前提,明确退药情形和处理要求,通过加强内涵建设,加强ADR/ADE报告和监测工作,建立临床药师咨询服务制度来提高我院合理用药的水平。

Reason Analysis and Countermeasures of Drug Repercussion in Primary Hospital

To find the management measures which are in line with the situation of our hospital, and to improve the level of rational drug use. METHODS： 180 drug repercussion applications collected from outpatient western medicine dispensary of our hospital during Aug. 23, 2010--Nov. 3, 2010, involving 367 kinds of drugs, were analyzed in terms of reasons, type of drugs, storage conditions with sections as the benchmark. RESULTS： Drug repercussion mainly occurred in pediatric departments, mainly due to drug adverse reaction or event （ADR/ADE）, condition changes, irrational drug use, etc. Those reasons accounted for 41.2%, 26.3%, 19.3% in drug repercussion frequency, a total of 86.8%. The type of drugs was western medicine which was stored at room temperature. CONCLUSION： On the premise of drug quality, clear drug repercussion situation and processing, enhancement of connotations construction and ADR/ADE reporting and monitoring, the establishment of consultation service system of clinical pharmacists in our hospital can improve the level of rational drug use.
