产科因素对产后早期盆底肌力的影响

摘    要:目的观察并探讨不同产科因素对产妇产后早期盆底肌力的影响,为产妇产后快速恢复提供临床参考依据。方法选择2012年3月-2013年7月期间在柳城县妇幼保健院妇产科分娩并行盆底功能检查的90例产妇为研究对象,根据分娩方式不同将其分为阴式顺产分娩的对照组和剖宫产的实验组,各45例。对比两组肌纤维肌力值与产科因素影响,并作统计学分析。结果两组Ⅰ、Ⅱ类肌纤维肌力值比较差异无统计学意义(P〉0.05);但实验组产妇Ⅰ、Ⅱ类肌纤维受分娩方式影响程度比较差异具有统计学意义(P〈0.05);对照组Ⅰ、Ⅱ类肌纤维肌力值比较差异无统计学意义(P〉0.05);新生儿体重和产妇年龄对实验组肌纤维肌力影响无统计学意义。结论剖宫产能够降低产妇产后盆底肌肉Ⅱ类肌纤维收缩能力因分娩而受到的影响,但无法确认是保护盆底结构不受分娩所致的损伤,因而单纯为保护产妇盆底功能而通过剖宫产结束妊娠在临床看来并不可取。

Abstract: Objective ] To observe and explore the early postpartum pelvic floor muscle strength under the effect of different factors of delivery, to provide clinical guidance in the rapid restoration of maternal postpartum. Methods ] In 2012 March to 2013 year during July in our hospital 90 cases of maternal pelvic floor function tests in parallel as the research object, according to the different modes of delivery will be divided into the control group and the experimental group, the control group experimental group for vaginal strength delivery, cesarean section, compared two groups of muscle fiber value and the obstetric factors, and for statistical analysis. Results ] There is no significant difference between the two groups Ⅰ ,Ⅱ muscle fiber strength, has no statistical significance (P〉0.05); the experiment group Ⅰ , Ⅱ muscle fibers were affected by the significant difference exists between the influence of mode of delivery (P〈0.05); the control group Ⅰ , Ⅱ type muscle fiber strength values showed no significant difference (P〉0.05); abnormal had no significant effect on body mass and maternal age newborns in experimental group muscle fiber strength. Conclusion ] Cesarean section can reduce the effects of postpartum pelvic floor muscle contractility of muscle fiber due to childbirth, but no proof is to protect the pelvic floor structure is not subject to birth injury, therefore simple to protect maternal pelvic floor function and by the end of cesarean section pregnancy in clinical opinion is not desirable.
