桥小脑角肿瘤的显微外科手术治疗

目的：总结桥小脑角肿瘤的临床特点和治疗体会。方法：回顾性分析本科1996年6月至2004年7月收治桥小脑角肿瘤36例的临床资料。结果：36例桥小脑角肿瘤中，听神经瘤28例，脑膜瘤4例，胆脂瘤3例，血管网状细胞瘤1例。本组均采用单侧枕下乙状窦后入路，运用显微外科技术进行肿瘤切除，结果肿瘤全切除32例，次全切除4例。全组面神经解剖保留32例（89％），功能保留27例（75％），术后永久性面瘫10例。结论：单侧枕下乙状窦后入路显微外科手术适用于不同性质的桥小脑角肿瘤，严格按照显微外科技术操作，肿瘤可全切除，并能有效地保护桥小脑角周围的重要组织结构及其功能。

Microsurgical Excision of the Tumors in Cerebellar Pontine Angle Region

Objective:To summarize the clinical character and therapeutic experience of tumors in cerebellar pontine angle (CPA) region. Methods: 36 patients with cerebellar pontine angle region who admitted to the department from June 1996 to July 2004 were analyzed retrospectively. Rusults: In 36 cases of CPA tumors, there were 28 cases of acoustic neuromas, 4 cases of meningimad, 3 cases cholesteatomad and 1 of angioreticuloma. All CPA Tumors were excised with microsurgical operation by unilateral suboccipital-retrosigmoid sinus approach. By using microsurgical techniques, the CPA tumor were totally resected in 32, subtotally resected in 4. With anatomically preserved facial in 32 cases (89%), and full function was maintained in 27 cases (75%), 10 patients appeared permanent facial palsy after the operation. Condusions: Unilateral suboccipital-retrosigmoid sinus approach combined with mierosurgical techniques is suitable for CPA tumors. The tumors could be completely removed by microsurgical operation and the surrounding structures of CPA area and their functions could be preserved.
