血清IL-2、IL-8及TNF-α水平在2型糖尿病肾病患者中的变化及意义

目的探讨2型糖尿病肾病患者血清IL-2、IL-8及TNF-α水平的变化及其在2型糖尿病肾病病理生理中的作用。方法2型糖尿病110例,其中糖尿病肾病72例,分为早期肾病（DN1）组35例,临床肾病（DN2）组37例,2型糖尿病无白蛋白尿组（DM）38例,健康对照组3O例;分别测定血清IL-2、IL-8及TNF-α水平。结果2型糖尿病肾病患者组血清中IL-2水平地低于正常人组及2型糖尿病非肾病组,而IL-8及TNF-α水平高于正常人组及2型糖尿病非肾病组,且随糖尿病肾病的加重差异更为显著;相关分析发现,IL-2与尿白蛋白排泄率（UAER）呈负相关,IL-8及TNF-α水平与UAER呈正相关。结论在2型糖尿病肾病的病理生理过程中,IL-2、IL-8及及TNF-α水平其发生的重要炎症介质,在2型糖尿病肾病的发生及发展过程中可能起重要作用。

Study on the changes of serum IL-2, IL-8 and TNF-α levels in patients with diabetic nephropathy

Objective To investigate the changes of serum IL-2, IL-8 and TNF-α levels in patients with diabetic nephropathy. Methods 110 patients with type 2 diabetes were selected for this study. The patients were divided into three groups according to urinary albumin excretion rate （UAER） ： 38 patients with normal UAER were DM group. 35 patients with microalbuminuria were DN1 group and 37 patients with clinical protein uria were DN2 group. Thirty healthy subjects were used as control group. The serum levels of IL-2, IL-8 and TNF-α were measured with was measured enzyme linked immunosorbent assay. Results Serum levels of IL-8 and TNF-α were significantly higher in patients with diabetic nephropathy than those in controls（ P 〈0.05 ,P 〈 0. 01 ） , but serum IL-2 levels were significantly lower in the patients （ P 〈 0. 05, P 〈 0. 01 ）. Conclusion These cytokines participated in the pathogenesis of diabetic nephropathy. Monitoring the changes of their serum levels was helpful for the management of the disease.
