高龄股骨颈骨折行髋关节置换治疗的临床疗效评价

目的:探讨采用髋关节置换治疗高龄老年患者股骨颈骨折的疗效,为此类患者股骨颈骨折的临床治疗提供参考。方法:回顾我院从1995年9月至2003年12月治疗的21例股骨颈骨折高龄患者(>70岁),双极股骨头置换15例,全髋置换6例,全部患者均采用骨水泥固定。结果:21例患者中,死亡6例,随访1年3个月～8年2个月,平均随访5年3个月,无髋关节感染及术后行走引起的脱位。两例双极股骨头置换患者行走时股前区轻度疼痛,一个月后缓解。疼痛发生率为1.4％。结论: 高龄老年患者股骨颈骨折双极股骨头置换术是较好的选择,可减少手术时间和并发症。

Clinical Evaluation of Bipolar Femoral Head Replacement and Total Hip Replacement Effects on Treatment of Femoral Neck Fractures in the Elderly

Objective: To evaluation the effects of bipolar femoral head replacement and total hip replacement on treatment of femoral neck fractures in the elderly, giving better evidences for treatment these patients. Methods: Bipolar femoral head replacement and total hip replacement operation were made from 1995 to 2003 in 21 elderly patients(>70 years), 15 cases were treated with bipolar femoral head replacement and 6 cases with total hip replacement. Cement were used in all of cases. Results: 6 patients died in 21 cases (from 10 months to 4 years), 15 patients were followed up 5 year and 3 months. No infection or spontaneous disloction occur. Thigh pain in 2 patients with bipolar femoral head replacement were found, and released after on month. Conclusion: Bipolar femoral head replacement is better choice for the elderly patients and rates of complication were decreased result from shorter operation time.
