| 摘 要： | 目的评价血管内栓塞技术治疗胰十二指肠术后大出血的安全性和疗效。方法对16例外科术后大出血患者进行了介入治疗。栓塞材料用微型钢丝圈10例,普通钢丝圈2例,同时用聚乙烯醇微球6例,同时用明胶海绵4例。结果选择性血管造影显示,单纯假性动脉瘤形成8例,合并造影剂外溢4例,单纯造影剂外溢4例。介入治疗止血成功率87.5%,再出血率14.3%。14例介入治疗成功者,术后经输血、补液等对症处理后,生命体征平稳,引流管及胃管引流液血性液体消失。其中3例合并胰瘘、腹腔脓肿者术后2周再次大出血、失血性休克而死亡,其余11例随访3周～25个月过程中,未再出现大出血。4例行肝固有动脉栓塞者ALT出现一过性升高,其他患者未出现胰腺坏死、脾脓肿表现。结论经导管血管内栓塞术是治疗胰十二指肠术后大出血的安全有效的方法。
